Responses to Written Questions
Q1) On page 7 of the RFP, Section II (CALENDAR OF EVENTS), are the Evaluation Period and Vendor Interviews dates supposed to be in August and not July?
A1) Please see the revised Calendar of Events on page 2 of this Addendum.
Q2) On page 5 of Exhibit A, Bid Form, the Unit of Measure is “each.” Can the County confirm that “each” refers to individual or per test?
A2) Yes, the unit of measure is per individual tested.
Q3) Will there be SLEB participation requirements for this contract?
A3) No, the SLEB requirements have been waived because there is Federal funding involved.
Q4) Where can bidders get a copy of the bid form in Word document format?
A4) The bid form can be found in the RFP. Vendors can download the RFP as a Word document from the County’s website at www.acgov.org under “Current Contracting Opportunities”.
Q5) What is the current fee schedule being paid by the County for these services?
A5) The County currently pays $38 per individual tested.
Q6) What is the current collection schedule for the vendor provided sites?
A6) Scheduling at the LabCorp site in Fremont is done on a per-request basis – County staff sends an email to LabCorp and are provided with the date/time for the collection. At the Oakland SLEB vendor site (see Q7 for SLEB information), collection is scheduledby County staff 8:30 a.m.-3:00 p.m. the 2nd and 4th Thursdays of the month.
Q7) Can the County provide a listing of the sites currently being utilized for collections?
A7) Fremont – LabCorp site. Oakland – SLEB vendor site: Accurate C & S Services, Inc., DBA Accurate Drug and DNA Testing – 8105 Edgewater Dr., #225, Oakland, CA 94621
Q8) Are samples collected at any DCSS location in Alameda County? If so, please provide the location and schedule.
A8) No, samples are not collected at any DCSS locations in Alameda County.
Q9) In regard to references on page 6 of Exhibit A, is supplying an email address in lieu of a fax number satisfactory?
A9) Please see the revised Exhibit A – Bid Response Packet on page 3 of this Addendum.
Q10) In regards to the financial statement listed on Page 8 of Exhibit A, can a D & B Comprehensive Insight Plus Report be submitted in place of a Supplier Qualifier Report?
