Primary Objective:
Implementation and coordination of the HARP program for CAH including budget, network, development, client assessment, care planning implementation and evaluation, data collection and reporting.

  1. Clinical Skills
Ability to undertake some clinical components of case management. /
  • Ability to undertake an initial assessment of clients and accept referrals based on relevance to the HARP guidelines.
  • Have a broad knowledge about the suite of services available to a client and actively support the clinical coordinator in referrals and advice.
  • Able to manage an agreed case load if required particularly in the first year whilst the program is developing.
  • Demonstrate the ability to achieve self management sustainable programs for clients.

  • Monthly summary provided to the Community Nursing Manager detailing current activity, identify obstacles and risks, and to present program improvement opportunities.
  • Participation in the relevant accreditation standards including the Community Care Standards and ACHS EQUIP guidelines.
  • Security risk assessments are completed as part of the admission process for new clients.
  • Participation in strategic planning activities.
  • Annual presentation at the Community Forum/ Leadership meeting on HARP and its outcomes.
  • Provide and review quality outcomes statements which form part of the Community Nursing Quality Action Plan.
  • Provide a written summary for Clinical Governance in preparation for the Annual Quality of Care Report.
  • A ‘quality’ component forms part of the manager monthly reporting requirement.

/ Qualifications, Experience and Personal Attributes –
  • Experienced Registered Nurse or Allied Health Professional Grade 2(social work, physiotherapy, or OT) in Victoria.
  • Evidence of Project management experience including the preparation of submissions and program evaluation.
  • Ability to work autonomously
  • Advanced assessment skills and relevant experience with chronic
disease management
  • Ability to articulate the HARP objectives.
  • Preparedness to participate in ongoing staff development to support the role
  • Experience working with community service providers, allied healthand GP’s
  • High level of interpersonal skills and emotional intelligence
  • Competent Computer literacy Skills
  • Current Police Check
  • Current Driver Licence
