Scope

The post holder will work between the Bristol Royal Infirmary and Southmead Hospital on a rotational basis or as required. They will be based primarily within the A&E department but will be required to travel throughout the Bristol, North Somerset & South Gloucestershire region in order to support vulnerable adults home from hospital.

The post holder will support the Co-ordinator in the delivery of the British Red Cross A&E Assisted Discharge service (BRC ADS); which involves the transportation and resettlement of vulnerable patients from hospital to their home. The hours of this post will be from 4pm until Midnight on a two weekly rotating shift pattern. Service hours may increase in future in order to meet changing service needs. The service operates 7 days a week; 365 days a year and the main aim of this service is to prevent and reduce hospital admissions.

Overall Purpose of the Post

As post holder you will be required to:

Be directed by the Co-ordinator to transport and resettle service users back into their homes following a visit to A&E and/or a stay in hospital.

Be based in the A&E department at BRI/Southmead in order to act as ‘first responder’ to referrals. This will involve transporting service user’s home in your own vehicle and resettling them back into the community.

Support the Co-ordinator to promote the service within the hospital bases and provide high quality resettlement care packages to ensure the service user feels safe and secure back in familiar surroundings.

Act as a first point of contact at the hospital and help co-ordinate the response to referrals when required (in the absence of the Co-ordinator).

Support the operational requirements of the service by assisting with / taking responsibility for the day-to-day administrative tasks required, such as maintaining rotas, dealing with incoming mail correspondence etc.

This is a role which can be unpredictable and requires a motivated, committed and enthusiastic person with good communication skills and an ability to deal with people in a sensitive and professional manner.

Main Duties and Responsibilities

  To support the Service Co-ordinator (SC) in fulfilling the contractual obligations of the project.

  Be based primarily in the A&E department of the BRI/Southmead but to also ensure service provision to all wards across these hospitals and others in the region, i.e. Bristol Heart Institute, Bristol Haematology & Oncology Hospital.

  To regularly work with minimal supervision with limited access to immediate support/advice

  To contribute towards the Daily Handover and Commissioners reports as requested by the SC in order to ensure that key information is captured and relayed as necessary.

  To act as shift co-ordinator on occasions where the Service Co-ordinator is not available, taking responsibility for the requirements of the service that evening.

  To ensure all paperwork is recorded and kept updated and submitted in a timely manner.

  To adhere to the BRCS confidentiality and safeguarding policies.

  To undertake training as required.

  Be directed and line managed by the SC.

  Work flexibly to enable staff team meetings to occur.

  To uphold the Fundamental Principles of the Red Cross and Red Crescent Movement and to work within the BRCS Equal Opportunities Policy.

  To undertake any other relevant duties requested by the line manager.

Education/ Training

/   Educated to GCSE level or equivalent**

Experience

/   Experience of working in the care profession
  Experience of working with vulnerable people**

Knowledge/ Technical Skills

/   Knowledge of British Red Cross Health and Social Care services - or willingness to undertake training
  IT literate. Experience of using email, word processing and spreadsheet packages**
  Knowledge of Safeguarding legislation, Health & Safety practices and Lone Working policies - or willingness to undertake training
  Ability to identify and assess people’s needs**
  Ability to promote the Assisted Discharge service and to raise its profile**
  Understanding of monitoring and reviewing service standards and quality**
  Ability to contribute to the development and achievement of operation/functional plans
