Members of the Public:

No members of the public were present.

I. Call to order/Introductions:

Ms. Vicki Pegram called the meeting to order at 5:36 PM. Introductions were made by members and affiliates.

II. Approval of Agenda:

A motion was made to approve the agenda was approved as written without any amendments by Mr. Parker and seconded by Ms. Guide.

III. Approval of Minutes:

A motion was made by Mr. Parker to approve the meeting minutes of July 8, 2014 as written and seconded by Mr. Moran and was unanimously approved by the committee without further discussion.

IV. Public Comment:

No public comments were made.

V. Old Business:

-The recorder of the minutes is to provide hard copies to the members by mail. This can be mailed with the quarterly report.

-The date of the Southeast Alliance Local Human Rights Committee from the 2nd Tuesday of the scheduled meeting date changed to the 3rd Tuesday of the scheduled meeting date. January 20th will be the next Southeast Alliance Local Human Rights Committee meeting.

VI. New Business:

Hampton – Newport News Community Services Board: H-NNCSB is requesting affiliation of intensive card coordination for children and adolescents. Ms. Iwaniw, Mrs. Ferrson- Powell, and Mrs. Smith presented information regarding the program to the committee. A motion was made by Mr. Parker to accept affiliation for this program and seconded by Ms. Guide.

Family Behavioral Health and Healing- Mrs. Swittenberg was informed that she could not request affiliation for new services of Therapeutic Day Treatment or proceed until she gets a letter from Central Office.

Second Chances requested affiliation for service medication for mental health skill building service but was informed by Mr Jones that they were already affiliated for this service in Region V, and a new affiliation was not necessary for the same service in the same region.

VII. Advocates Report:

-Mr. Jones announced that Deb Lochart was appointed as the State Human Rights Director.

-Mr. Jones informed the affiliates to mail quarterly reports and do not send them by email.

-Mr Jones informed the affiliates that there is a difference when adding new services versus adding a new site. New services requirethat the human rights verification form and behavior policies and procedures must be sent to Mrs. Deb Lochart. Once the provider receives the letter from Ms. Lochart you are to send a copy of the letter your human rights policies, behavior management policy and program rules to the Office of Human Rights and the LHRC members. If adding a new site for a service that the program already providers, the provider is to mail out a letter addressed to the LHRC chair with the Office of Human Rights and the licensing specialist being CC. The letter needs to include the address of the new location and indicate that same policies and procedures used for affiliation will be used.

Mr. Jones expressed the importance of sending the quarter reports prior to the meeting. Quarter reports will not be accepted at or during the meetings.

  • Family Behavioral Health and Healing – Ms. Swittenberg presented and reported. Need to used updated Quarterly Report Form 0 abuse case, 0 complaints
  • H-NNCSB – Ms. Iwaniw presented and reported. 3 abuse cases, 3 complaints
  • Integrated Health Service – Ms. Mitchell presented and reported. 0 abuse case, 4 complaints
  • Lutheran Family Services of Virginia- Ms. Stephens presented and reported. 0 abuse case, 0 complaints
  • Second Chance Youth Services – Ms. Ward presented and reported. 1 abuse case, 0 complaints
  • Together Lives Change – Ms. Wright presented and reported. 0 abuse case, 0 complaints
  • VersAbility Resources – Ms. Palmer presented and reported. 1 abuse case, 0 complaints
  • Gray’s Family Services – Ms. Valerie Gray presented and reported1 abuse case, 0 complaints

IX. Closed Session:

A motion was made by Mr. Parker and seconded by Mr.Moran at 6:39 PM that the SEA - LHRC go into Closed Session pursuant to Va. Code § 2.2-3711(A paragraph 4), for the purposes of protecting the privacy of individuals in personal matters not related to public business, namely for the purpose of hearing from Colonial Behavioral Health, VersAbility Resources, Hampton- Newport News Community Services Board, Gray’s Family Services,Integrated Health Service, Together Lives Change, and Second Chances Youth Services pursuant to the regulations.

A motion was made by Mr. Parker and seconded by Mr. Moran at 7:26 PM to exit closed session.

X. Open Session:

Upon reconvening in open session, each member of the SEA LHRC certified to the best of each member’s knowledge, only private business matters lawfully exempted from statutory open meeting requirements and only private business matters identified in the motion to convene the executive session were discussed in executive session.

XI. ADJOURNMENT:

There being no further business, Ms. Pegram - Chair adjourned the meeting at 7:28 PM.
