Section 1 – Parental/ Guardian Help

By registering a child with the club, all parents agree that they will help with one or more of the below activities on a regular basis:

1) setup and take down goal posts (both for practice and matches)

2) act as linesman (in relevant age groups)

2) help line fields 1-2 times a year

3) work on the CJFC Committee

5) help coach.

Section 2 – Disclaimer of liability

1. CJFC assumes no responsibility or liability for any injury/ damage to any person,persons or property whether resulting from taking part in any activity of the club or otherwise.

2. CJFCwill use all reasonable endeavours to contact the parent/ guardian or emergency contact in theevent of illness or injury.

Section 3 – Club Code of conduct

CJFC is a Charter Standard Club.

Please remember that parents and coaches give up their free time to help so all players must show respect. Inappropriate behaviour will result in being barred from the coaching/ matches at the club’s discretion. CJFC follows the FA “Codeof Conduct” for players, parents and spectators which needs to be signed by both CJFC player and theirparent or guardian.

The player and parent must sign the player form. A parent must sign the parent and spectator form. These can be foundon the FA Web site, the CJFC web site or can be obtained from your coach. They can be emailed to you if requested.

Section 4 – Photography

CJFC follows the FA recommended guidelines on photography. These can be found on the FA Web site, the CJFCweb site or can be obtained from your coach. By registering a child with the club, parents (i) acknowledge that it is notan offense to take appropriate photos in a public place, even if asked not to, and that no one has the right to decide whoand who cannot take photos on public land; (ii) understand, acknowledge and agreethat photos of their child may be taken in the course of playing for CJFC; and (iii) agree that photos of their child may be used by the club on the club website.

Section 5 – Data protection

The club will use the information provided on this form (together with otherinformation it obtains about the player) to administer his/her footballing activity at the club and in any activitiesin which he/ she participates through the club and to care for and supervise activities in which he/ she isinvolved. In some cases this may require the club to disclose the information to County Boards, Leagues and theFootball Association. In the event of a medical issue or a child protection arising, the club may disclose certaininformation to doctors or other medical specialists and/ or to police, children’s social care, the courts and/orprobation officers and potentially to legal and other advisers involved in an investigation. By registering a child with the club, parents agree to this use of personal data about the parent and the child.
