Health Checks: Offered to all patients aged 40 and above.

Long Term Conditions Clinic:

Inc. Diabetes / Asthma / CHD / COPD reviews. Immunisations (inc Childhood & Travel)

Smoking Cessation

Weight loss & Lifestyle Management

Child Health Checks

Minor Surgery: Cryosurgery, Joint Injections

Contraception: Emergency Contraception

Cervical Screening

Multi-Disciplinary Team Mettings: Held with other service providers reviewing palliative care & safeguarding issues.

Antenatal care: provided by Community Midwives

Physiotherapy: provided by Community Physio

Registering at the Practice

To register as a patient, you must live within the practice area (see map – within the dark line). You will need a pre-reg medical with anHCA prior to being seen by the doctors. (Please bring a copy of your medication list.

3 forms of ID are preferable).

Location: Northway Medical Centre is situated in a shopping precinct off The Northway which has exits onto Gospel End Road and Wolverhampton Road East. We normal accept patients within a 2-mile radius of the surgery. For the exact Practice Area see map (above) or enquire at Reception.

Disabled Access:

The premises are fully accessible to patient in wheelchairs with a disabled drop-off by the main entrance

Health

Home Visits:

Emergency visits, consultations and routine visits arranged via Receptionist. For home visits please telephone before 10.30am. Home visits should be for severe illnesses or the truly bedbound patients and not for illnesses such as flu or back pain.

Repeat Prescriptions:

2 working days notice required.

Prescriptions to be requested online via Patient Access Services or in person at Reception.

Consultations:

* Are by appointment for one person only

In an emergency, you can ask to see a doctor urgently, on the same day.

* If you have an urgent problem and an appointment is not available with the doctor of your choice, one will be offered with whichever doctor is free

* Please cancel all appointments which you no longer need as these can be offered to other patients

* If you arrive at the Practice more than 15 minutes late, you will be asked to rebook your appointment.

* Patients between the ages of 16 & 75, and aged 75 & over who have not been seen by a doctor or nurse for any reason during the last 3-years will be offered a consultation if requested by the patient. Patients over the age of 75 who have not been seen within the last 12-months should make an appointment for a health check.

* Where appropriate, it is possible for you to speak to a doctor over the telephone. Please contact reception for further information. A telephone consultation IS NOT a substitute for a face-to-face consultation, and Reception may inform you that you will need to be seen at the surgery

Complaints

Any comments or complaints that you may have concerning your care at the Practice can be made to the Practice Manager in person by a prior appointment or in writing
