DSA College Application Documentation Checklist

_____ Transcript Request Form (opposite side of this sheet.)

_____ Two addressed, stamped envelopes for each college. (one for the current transcript and one for the midyear transcript.)

*After January 1, 2013, Ms. Vliet will only need one stamped addressed envelope, as midyear grades will be on the transcript.

_____ Paper College Application (If you did not apply online.)

_____ Secondary School Report (If you filled out the Common Application “invite” your counselor to fill out the form online instead.)

_____ A filled out, pink Letter of Recommendation Request form, which you can find in the Counseling Office

_____ Any miscellaneous forms to be filled out by counselor (Your application will specify if there are specific forms for your counselor to fill out. If there are, you must print them out and give them to your counselor.) It is your responsibility to check with your colleges to see what additional forms they need.

_____ Payment for transcripts. (See Ms. Vliet for amount you will owe.)

1.)  Beginning of year and mid-year transcripts for the first school are free. The cost is $6.00 per school for all other requests, ($3.00 for current and $3.00 for midyear). After the first week in January, the cost of transcripts will be $3.00 per school, because midyear grades will then be posted. Each time you request a transcript, you must pay Ms. Vliet.

2.)  If you would like your teacher to write you a letter of recommendation, give them a stamped addressed envelope to mail the letter or form to your school. This should be handled directly with your teacher, not through the Counseling Office. If you are using the Common Application “invite” your teacher to fill out the form online, no envelope is necessary if this is the case. Don’t forget Thank You Cards for teachers who wrote letters of recommendation for you.

3.)  Please do not put Transcript Requests or other documents on Ms. Vliet’s desk. It is important that you give your request directly to her.

Note: Ms. Vliet has envelopes stamped with the school’s return address and she also sells stamps.
