Re: END OF YEAR CHECKLIST (Please read each item completely! All are important!)

You will need to have each step signed by the appropriate person

Please be sure you have

___ turned in a hard copy of grade book: assure all grades are clearly indicated including each six weeks, both semesters and the final grade (PK thru 6th Mrs Hagle , 7th thru 12th Mr. Johnson )

___ turned in list of students who failed (a) last six weeks, (b) semester, (c) year; or (d) is being retained (PK thru 6th Mrs Hagle , 7th thru 12th Mr. Johnson )

___ turned in your booklist: indicate who turned in books and who did not; High School & Junior High also confirm that you have fully accounted for the classroom set(s) of textbooks. Be sure you check book numbers! (Opal Moore)

___ turned in completed yearly textbook count form: be sure to include your ending year textbook count (high school office) (Opal Moore)

___ turned in any textbook needs for next year: (high school office) (Opal Moore)

___ turned in a copy of DIBELS report to be filed in each student’s permanent folder and class summary to Mrs. Hagle (K – 2nd)

___ boxed and labeled all ELA textbooks according to instructions previously emailed by Mrs. Opal

___ make sure all grades are marked “ready to post” in gradebook (Susan Higginbotham)

___ turned in copy of final exams (high school) (Opal Moore)

___ turned in your summative observation form (Daniel Johnson, Heather Hagle)

___ turned in your repair list (any repairs, classroom needs, etc) to Mr. Gordon (Gordon Alvis)

___ turned in your student supply list to Mrs. Susan (PK-8th) (Mrs. Hagle/Mr. Johnson)

___ provided two stamped self addressed envelopes to Ms. Jeanne (only if you do not have direct deposit—if no envelopes are provided, your check will be placed in your mailbox) (Jeannie Holloway)

___ turned in all sick leave forms to Ms. Jeanne (Jeannie Holloway)

___ signed up for 3 summer workshops: proof of workshops must be submitted at the beginning of 2010-2011 school year to receive credit for comp days (Mrs. Hagle/Mr. Johnson)
