Updates to FOQIS Materials for ASPEN 10.1.5/QIS 4.02

  1. FOQIS Procedures
  1. General comment – Changed all QTSO website references to S&C.
  2. Step 1 – added Resident Observation to Dignity
  3. Step 7 – updated reference to the IT Manual
  4. Step 9(1) – clarified that the FOQIS is completed for a recertification survey, not a certification survey.
  5. Step 14 – Changed wait time to 90 seconds. Changed QTSO to S&C.
  6. Step 15 – Added guidance on how the RO should handle disagreements with the SA regarding FOQIS findings (e.g., Stage 1 disagreement) and IJ. Used the same language that is in the FOSS manual.
  7. Step 16 – Deleted reference to Unnecessary Meds and initiated care areas not being displayed on the Stage 2 Report by Stage 2 Surveyor (this was fixed).
  8. Step 20 – Included that the RO can provide guidance and education.
  9. Step 22 – Removed the text requiring a State Team Action Plan. Replaced the text with, “Any concern should be provided to the SA manager so the State can follow-up internally.”
  10. Step 24 – Deleted reference to sending scoring sheet to CMS and CU.
  11. Step 25 – Deleted entire step since it was related to the State Team Action Plan.
  12. Step 26 – Renumbered to Step 25. We deleted the work-around steps and made a general statement to enter the FOQIS results in the FMS database.

  2. Removed the text requiring a State Team Action Plan under each measure. Replaced the text under each measure with, “Any concern should be provided to the SA manager so the State can follow-up internally.”

  22. We also changed the 1st activity response from No to Yes since that had not been considered a negative response in the activity.
  23. Under step 6 in the activity, we deleted the abuse issues since the questions were changes.

  25. Deleted reference to ROs being embedded with the SA team.
  26. Updated the wait time from 60 seconds to 90 seconds to be consistent with the procedures.
  27. Added the third suggestion for a workaround if the cable doesn’t work (i.e., email the file to the RO).

  29. Updated the threshold for the Dignity info (from 5 to 5.8) and updated all associated calculations.
