Project Owner:

State Aid Project No.:

Bridge No.:

For the above referenced project, [Project Owner] intends to use wood preserved with the following preservatives:

·  Structural Components – [Micronized Copper Azole (MCA)] [Copper Naphthenate (CuN)] per AWPA Use Category UC4C

·  Decking – [Micronized Copper Azole (MCA) per AWPA Use Category UC3B] [Micronized Copper Azole (MCA) per AWPA Use Category UC4A] [Propiconazole Tebuconazole Imidacloprid (PTI) per AWPA Use Category UC3B] [Copper Naphthenate (CuN) per AWPA Use Category UC3B] [Copper Naphthenate (CuN) per AWPA Use Category UC4A]

·  Railing – [Micronized Copper Azole (MCA)] [Propiconazole Tebuconazole Imidacloprid (PTI)] per AWPA Use Category UC3B

[Project Owner], understands that this preservative treatment may not conform with the MnDOT approved preservatives for the treatment of timber products and the restrictions established with regards to proximity to [surface water][residential or recreational areas].

[Project Owner] recognizes however, that the wood preservatives proposed on the Project have been adopted by the American Association of State Highway and Transportation Officials (AASHTO) and have been reviewed and approved by the US Environmental Protection Agency for this project application. The preserved wood will be certified in accordance with American Wood Protection Association Standards for use, and will be dried to the seasoning requirements found in MnDOT specification 3491.2 B.1.d.

[Project Owner] considered factors such as environmental risk, cost, and durability in their final selection of wood preservative for this local timber bridge project. [Project Owner] also understands they can discuss the potential environmental liability associated with their selection of wood preservative with the MnDOT Office of Environmental Stewardship (651-366-3617) at any time.

[Project Owner] believes it is in the best interest of the Project to use these preservatives and agrees to accept the responsibility for the effects of their use.
