Cost of vendor space is: _____ $60 for cupcake vendors and a donation of (1) half

dozen of cupcakes to be used for pageant entrants – spaces

will be 12X8 spaces

Deadline to register for a space is Friday, August 10, 2018.

For the 2018 festival, we will have 14 spaces available. The cupcake vendors from our 2017 Festival will be contacted with first opportunity to reserve the space for this year. They also will have a deadline of April 23, 2018 to submit applications and fee to reserve a space. If he/she does not reserve the space by this deadline, the space(s) will become available on a first come/first serve basis. Once a vendor notifies us of his/her interest in a space, he/she will have10 days to submit registration form, health department form and fee. After 10 days, that space opens to the next in line if we have not received documents and payment. Once the 14 spaces are filled, we will notify everyone via a Facebook post and it will also be posted on our website.

Forms of payment accepted: Cash, Check or Money Order (Payable To: Rita Skeens) or PayPal (additional $2 processing fee will be added to PayPal payments and completed registration form must be emailed at time of payment).

All cupcake vendors must fill out a form to be submitted to the health department. Please send this form to us with your registration and we will send them to the health department prior to the event. The link to form can be found on our website just below the link for the application. There is also a link stating the rules and regulations that must be followed as a cupcake vendor. These are rules required by the Putnam County Health Department in selling baked goods at fairs and festivals.

I have read and understand all regulations provided by the Putnam County Health Department and will abide by these rules and regulations. I also understand that not following these regulations can result in not being permitted to participate as a vendor at the WV Cupcake Festival. I also understand that the WV Cupcake Festival is not responsible for any losses I incur or registration fees lost in result of not following these regulations.______

ALL CUPCAKE VENDORS ARE REQUIRED TO HAVE A MINIMUM OF 600 CUPCAKES AVAILABLE AT THE FESTIVAL. We have set this minimum due to demand at the festival and to ensure a successful event. ______

Cupcake vendors are also required to price the cupcakes at a minimum of $2 each. If any vendor wishes to bring more product or price his/her cupcakes at a higher price, he/she may do so. During the final 2 hours of the event, anyone wishing to reduce the price to ensure sell through may do so, but no earlier than 3 pm. ______

WE ARE NOT RESPONSIBLE FOR ANY PRODUCT THAT IS NOT SOLD. ______

All vendors must provide any tables, chairs or any other supplies needed. WE WILL NOT BE ABLE TO PROVIDE ANY TABLES, CHAIRS, ETC THE DAY OF THE FESTIVAL______

Vendors take full responsibility to any damages or loss of any of his/her merchandise, supplies, and/or personal belongings. ______

In signing this application, VENDORS AGREE TO STAY THE ENTIREITY OF THE FESTIVAL.If a vendor leaves prior to the conclusion of the festival, they will not be invited back in the future.______

THE FESTIVAL IS A RAIN OR SHINE EVENT. Registration fees are NON-REFUNDABLE if the vendor decides to cancel. However, if the event is cancelled for any reason, registration fees will be refunded. ______

I UNDERSTAND AND ACCEPT THE RULES AND REGULATIONS LISTED ABOVE. I WILL ALSO ALLOW MY PHOTOGRAPH AND NAME TO POTENTIALLY BE USED IN PUBLICATIONS OR SOCIAL MEDIA AS A RESULT OF THIS EVENT.
