If you are registered with an East Riding GP you can access this new NHS self referral service to talk to a physiotherapist without seeing your doctor first
To discuss your problem with a physiotherapist call:

The self-referral service is open to patients with an East Riding GP practice or Vale of York CCG
The physiotherapist will be able to advise you about your problem. You may be asked to attend a face to face assessment at a clinic of your choice or be referred for treatment if this is necessary. You may also be referred on to your GP or other health professional if needed.

The East Riding Health Trainer Service provides personal support and motivation to adults who wish to lead a healthier lifestyle.
Health trainers work with you on a one to one to help you set achievable goals, develop a personal plan, identify and overcome barriers and provide ongoing encouragement and support.

Active in Age:Active in age is a physical activity class designed to help improve mobility, co-ordination and balance.
Health Walks:Gentle, but fun and a great way to socialise. Walk length 30 minutes.
Health Chat:Healthchat training is a two hour workshop to provide information and facts and learn more about health topics such as obesity, smoking, physical activity, falls and alcohol awareness.
Health Checks:Offered to all patients accessing the service.
Weight Management Club:
A FREE, weight loss group where you can get practical, no nonsense advice and support to help make long term lifestyle changes.

TRAVEL VACCINATIONS
Please pick up a Travel Vaccination Questionnaire from the surgery. You should allow plenty of time before you travel for your travel vaccinations.
ELECTRONIC PRESCRIPTION SERVICE
This is an NHS service, it gives you the chance to change how your GP sends your prescription to the place you choose to get your medicines or appliances from.
Your GP will send your prescription electronically to the place you choose, saving you time. You will have more choice about where to get your medicines from and you may not have to wait as long at the pharmacy.
