Participation in the Sacramento Betsuin Basketball Program requires that both Participants and Parents/Guardians comply with the

SBC Sports Program bylaws, rules of participation, and the consent release clauses set forth below. Participants are required to:

§ Have their own medical insurance. The Betsuin Sports ProgramDOES NOT provide medical coverage.

§ To participate in Spring Jamborees, I-Leaguers must maintain a min. 60% attendance in Church Service AND Dharma School.

Participants/Parents shall display good sportsmanship, honoring the standards/objectives set forth by the Sacramento Betsuin Sports Committee. Participants/Parents are REQUIRED TO CONTRIBUTE to the Sports Program and/or Betsuin activities, as outlined below:

· Mochitsuki, Saturday, 12/16/2017 (i.e., working as a steamer, grinder, shaper, etc.); our Program’s only designated fundraiser.

· Annual Bazaar - 8/11 - 8/12/2018 - ALWAYS THE SECOND FULL WEEKEND IN AUGUST; PLEASE DO NOT SCHEDULE VACATION DURING THIS WEEKEND. Our Sponsor Organization’s most important fundraiser the ONLY SBC EVENT for which Sports Families’ support is requested by the SBC Board of Trustees. Proceeds enable us to enjoy the many benefits of the Sports Program and SBC facilities. Annual Bazaar duties require PARTICIPATION BY EVERY REGISTERED FAMILY, as follows:

a. MANDATORY: Sports Program work shift (player + one parent)

b. Bazaar Food Prep (Thurs, 8/9/18, afternoon/evening – SO MUCH HELP IS NEEDED!!) - HIGHLY ENCOURAGED!

c. Dharma School work shift (Assist w/Bussing Outdoor Tables) - HIGHLY ENCOURAGED!

A NON-COMPLIANT FAMILY PLACES AN UNDUE BURDEN ON THE PROGRAM. FAILURE TO MEET PARTICIPATION REQUIREMENTS MAY PRECLUDE A PARTICIPANT FROM FURTHER PROGRAM ACTIVITY. By my initials, I state that I have read and fully understand this Waiver Release of Liability, as well as my SBC Sports Program obligations. I voluntarily agree to the terms and conditions stated above.

As parents/legal guardian of the named Participant, I hereby authorize and consent to medical treatment deemed necessary and proper in the event that a delay in obtaining my consent at the time of treatment may be detrimental to the well being of the Participant. The care may be given under whatever conditions are necessary to preserve the life, limb, or well being of the Participant.

As parent/legal guardian of the named Participant, I hereby give my approval and consent to his/her participation in any and all activities sponsored and/or conducted by the Sacramento Betsuin Sports Program. I expressly hereby assume all risks and hazards incidental to such participation including, but not limited to, transportation to and from such activities; and I do hereby expressly waive, release, absolve, indemnify, covenant not to sue, and agree to hold harmless the Sacramento Betsuin, its trustees, its officers, its directors, the organizers, sponsors, supervisors of, and participants in activities sponsored and/or conducted by the Sacramento Betsuin, including persons transporting the Participant to and from such activities, Sacramento Asian Sports Foundation, Sacramento and Elk Grove Unified School District, for any claim, loss, damage, liability, medical costs, and attorney’s fees and legal costs arising out of any injury to the Participant, whether or not the result of negligence or any other cause.

By signature below, I certify that I am a parent or legal guardian of the named Participant, that I understand the nature of the basketball activities presented by the Betsuin Basketball Program and the risks inherent in such activities, and that I have read and understood the foregoing, and in consideration for the Participant’s participation in the Betsuin Basketball Program and related activities, I agree to comply with the rules of participation and to the consent and release clauses stated above.
