Please complete this form as an ELECTRONIC VERSION ONLY. Submit your completed assessment to the following email address;
1 / LOCATION OF THE WORK ACTIVITY / W1312 / PERSONS WHO MAY BE AT RISK
List names where possible / Antony Jones, Liba Klovorzova, Laine Wallace
3 / ACTIVITY ASSESSED / Use of Propan-2-ol
4 / MATERIALS INVOLVED
NAME and CAS # / AMOUNT
and FORM / HAZARD / RISK PHRASES / REPORTABLE?
Propan-2-ol / 200ml liquid / Highly Flammable / R11 : Highly Flammable / N
Irritant / R36 : irritating to the eyes
R67 : Vapours may cause drowsiness and dizziness
If a substance is reportable, have you reported it to the Health and Safety Unit? YES/NO (See note 4)
5 / INTENDED USE and JUSTIFICATION (where appropriate)Give brief details and attach protocol/instructions. Justification is needed for exceptionally hazardous substances (See note 5)
200ml Volume used for flushing the pump lines on the 3700 DNA analyser during a clean up procedure.
6 / RISKS to HEALTH and SAFETY from INTENDED USE
From personal exposure or hazardous reactions. Refer to WELs, flash points, etc., as appropriate. Are pregnant women, breast-feeding mothers especially at risk?
Risk of Fire. Flash point of 53°F, general use at room temperature is low risk.
Risk of irritation from splashes to eyes.
Risk of adverse effects from inhalation of vapour, WEL is 999mg/m3. Amount used is within OEL for procedure.
7 / CONCLUSIONS ABOUT RISKS
Is level of risk acceptable? Can risk be prevented or reduced by change of substance/procedure? Are control measures necessary?
Level of risk is acceptable.
8 / CONTROL MEASURES
Additional to Good Chemical Practice,e.g. fume cupboard, etc. Any special requirements, e.g. glove type, etc.
Extinguishing of all ignition sources before use.
The wearing of a lab coat, eye protection and nitrile gloves.

11 / WASTE DISPOSAL PROCEDURE
See School Server for Approved Procedure Document on specific Chemical Waste Disposal.
Include name, 6-digit code and H number if waste is the be disposed of by outside contractors (See note 11)
Treat as non-halogenated special waste and deposit with stores for specialist waste disposal using the codes:
H3-A
14 06 03
12 / REVIEW
Enter the date or circumstances for review of assessment (maximum review interval 5 years)

13 / EMERGENCY ACTION
TO CONTROL HAZARDS / To stabilize situation eg spread absorbant on liquid spill; eliminate sources of ignition, etc.
Extinguish allsources of ignition, be aware that vapour may travel some way towards sources of ignition.
Any hazardous waste generated from an emergency situation must be assigned a Type Code and Hazard Classification number(s) prior to the specialist disposal in accordance with the Hazardous Waste Regulations 2005.
Small spills should be mopped up with tissue, allowed to dry off in a fume hood and disposed of via the contaminated waste system.
Large spills – up to 200ml ( up to the WEL for the room) , eliminate all sources of ignition, ventilate the room by opening a window if possible. Whilst wearing heavy duty nitrile gloves, lab coat and eye protection Proceed with clean up by applying an absorbent material ( vermiculite), sweeping into a bag with non—sparking implements. Waste materials should be disposed of as special waste through stores.
Larger spills – above 200ml ( above the WEL for the room). Eliminate all sources of ignition and evacuate the room. Call the fire Brigade to facilitate clean-up.
TO PROTECT PERSONNEL / Evacuation, protection for personnel involved in clean-up, Special First Aid
If affected by vapour, remove to fresh air and seek medical advice if symptoms continue.
TO RENDER SITE OF EMERGENCY SAFE / Clean-up/decontamination
Cleanse the area with a dilute decon solution and dry thoroughly.
