The Practice is situated in Leslie, a small village near Glenrothes Fife.Leslie GPs are situated in Leslie Medical Practice next to the clinic where the District Nurses and Health Visitors are based.The Practice area is predominantly urban. It covers a wide geographical area including Leslie,all of Glenrothes, Thornton, Kinglassie, Markinch and all villages out as far as Loch Leven.

The present day building at Leslie replaced the old clinic, which stood on the same site, in 2008. It has been purpose built to provide accommodation for a Dental Practice, District Nurses, Health Visitors and Podiatrist.The Practice population presently stands at around 6200

The Practice has four General Practitioners, two practice nurses and a Health Care Assistant.

The District Nurses and Health Visitors have offices on the top floor of the Clinic. They hold case load notes which provide a more in-depth analysis of the health and nursing needs of particular patients/clients. They also have computers to which you will be allowed access.

There is an Information & Resources Centre based at Cameron Hospital where you would be able to obtain leaflets and fact sheets on various topics.

The District Nursing Team are committed to providing a positive learning environment in which we aim to support students to deliver safe and effective person centred care.

We acknowledge the quality standards for Practice placements and continually strive to meet them.

Our overall goal is that you achieve your Practice learning outcome in a supportive environment appropriate to your stage of training and that you gain insight into the Primary Health Care Team and its objectives.

Above all, we hope you enjoy the experience but should you have any concerns, we actively encourage open discussion.

PHILOSOPHY OF DISTRICT NURSING

The Philosophy of District Nursing believes that:

Patients/Carers are unique individuals with physical, emotional, spiritual, social and cultural needs living in their own complex environments. Patients/Carers will be enabled to be nursed and supported within their own homes from Community based Nursing Service.

Those individuals who temporarily or permanently require skilled professional nursing care and advice within the community will be offered a wide range of care. A holistic approach to care is given. Assessment will be given to all new patients by a District Nurse within 24 hours of referral.

Every patient will be given the name of District Charge Nurse responsible for his/her nursing care.

The influence of socio-economic, political and demographic factors on the planning care will be recognised and taken into account.

Potential patients and carers will have legitimate right to refer themselves to the District Nursing Service outwith the formal structure, for assessment of nursing needs. Any relevant information will be made available by the District Nurse to members of the Primary Health Care Team.

Patient/Carer participation in planning of delivery of care will be encouraged.

Health promotion and education of Patient/Carer will be the responsibility of the District Nurse. Patients/Carers will then be enabled to make informed choices and maximise their own health potential.

District Nurses draw upon a research based theoretical knowledge and experience to provide an advanced level of clinical nursing. They are proficient in the complex skills of personal relationships, management and teaching.

Each member of the nursing team is accountable for his/her own practice and will work in a collaborative and co-operative manner with other Health Professionals recognising their contributions (reference UKCC Code of Professional Conduct).

The Code of Confidentiality is of paramount importance in Community Nursing and must be observed by all members of the team (reference UKCC Booklet “Confidentiality”).

PHILOSOPHY OF HEALTH VISITING

“Health is a fundamental human right. The achievement of health involves rights and responsibilities which should be reciprocally shared between the individual and society”

Health Visiting is distinguished from other branches of nursing by the emphasis placed upon the pro-active search for health needs and preventative care. It is specifically directed to promoting, enhancing and preserving health. This is achieved by focusing on individuals and their families as members of the wider community.

The aim of Health Visiting is the improvement of Community Health by:

  • enabling clients to make informed choices about their own health and well-being
  • empowering individuals to utilise health and social services optimally
  • working in partnership with clients
  • identifying potential health stresses
  • offering anticipatory guidance
  • influencing social and political decisions related to health
  • responsibility for Child Protection. In accordance with Fife’s Child Protection Guidelines

Practitioners are autonomous members of the Primary Health Care Team. They work independently and as members of multi-disciplinary teams to plan, implement and evaluate health care programmes with their clients.

Health Visitors are bound by the NMC Code of Professional Conduct relating to accountability for practice, confidentiality and collaboration with other professionals.
