Waiver and Release form: Each individual archer and adult scorer must sign and return a copy of the Waiver and Release form. Make additional copies of registration and/or waiver forms as necessary for additional archers or adults.

Mailing: Each school must submit one registration form, attach all Waiver and Release forms (ensure these are completed by the parent or guardian!) and one check or money order for the total due for all registrants. Make the check payable to Texas Archery In Schools Program and mail to:

Refunds:

·  A full refund will be made for cancellations by close of business Monday, Feb. 8th, 14 business days before the tournament.

·  Half of the registration fee will be refunded for cancellations made after close of business Feb. 8th through close of business Feb 19th, between 5 and 14 business days before the tournament.

·  No refunds will be made for cancellations after close of business Friday, Feb 19th, 5 business days prior to the tournament date.
