The Kitchener Downtown Community Health Centre (KDCHC) is an interdisciplinary primary health care centre dedicated to working with diverse cultural groups, the homeless, youth seniors and others living in the downtown area of Kitchener.

KDCHC is a partner in the Community Diabetes Program, Waterloo Region. We are seeking a:

DIABETES EDUCATOR- REGISTERED NURSE FULL TIME

Position Description:

The Community Diabetes Program, Waterloo Region is an innovative community based education program. The program provides group and individual education and follow up to people who are newly diagnosed with low to moderate risk Type 2 diabetes. The Kitchener Downtown Community Health Centre is responsible for program and service delivery in Kitchener-Waterloo.

The program, which has been operational for 5 years, is currently staffed by two full time RN/RD teams. The successful candidate will work as part of the current teams to provide individual counselling and group education; presentations to community groups; outreach to at risk populations in the community; development of educational resources; development of a comprehensive evaluation process, participate in KDCHC staff meetings and program initiatives; perform administrative tasks and work collaboratively with partner CHC Diabetes program staff in the Region.

The following are the requirements for the position:

·  BScN from a recognized university or equivalent combination of education and experience

·  Certificate of Competence from the College of Nurses of Ontario,

·  3-5 years experience in a community setting or combination of community/hospital/ public health settings;

·  experience in primary care an asset;

·  Certified Diabetes Educator designation (if not currently held, it will be an expectation that a CDE designation is obtained as soon as possible).

·  Broad knowledge of diabetes management and principles of adult education including experience in counselling clients with diabetes

·  Available to work flexible hours based on programming needs.

·  Demonstrated counseling, problem-solving, and assessment skills.

·  Demonstrated ability to communicate (both verbally and in writing), and to deal effectively with co-workers, physicians, other health care professionals, clients, their families, and external agencies.

·  Experience in working with diverse population base, including multicultural populations, those experiencing homelessness, mental illness, those with substance use issues.

·  Demonstrated computer skills word processing; spreadsheets; power point presentations. Experience with electronic health records would be an asset

·  Experience in addressing determinants of health in a diverse community.

·  Demonstrated ability to work independently and in collaboration with others as a member of the interdisciplinary team.

·  Evidence of ongoing professional development, continuing education and leadership skills.

Salary Range: $55,116 to 66,405 plus competitive benefit package.
