Re: Freedom of Information Request

With reference to your Freedom of Information request relating to the Summary Care Record, please find below a response on behalf of NHS Hartlepool, NHS Middlesbrough, NHS Redcar and Cleveland and NHS Stockton on Tees.

Response:-

·  Please could you kindly state the total number of GP practices within each PCT that are uploading data to the Summary Care Record as of the end of Sunday 31st October 2010 - Middlesbrough PCT = 8, Redcar & Cleveland PCT= 8, Stockton-on-Tees PCT = 6, Hartlepool PCT = 6.

·  Please could you kindly state the number of practices within each PCT that have added more than core information to patient summary care records (that is, have enriched one, some or all of their patients' summary care records) - None of our practices have been enriching, however we have made it clear that enrichment of a summary care record is the decisionthe GP and the Practice make in consultation with the patient. The PCThave not mandated this process.

·  Please could you kindly state the total number of patient summary care records, as uploaded by participating practices within each PCT, that have been enriched in any way beyond core information - The enrichment of a summary care record is the decision a practice makes in consultation with a patient. The PCThave not mandated this process.
