JOB SUMMARY

The Case Management Practitioner will provide support, mentoring and training to youth program staff to build their knowledge and skills in the area of case management. This role will support the whole youth program across the tri-state area to ensure that client case management practices are in line with NPY Women’s Council practice framework.

The establishment of the position has sprung from an aspiration to create a best practice environment in which case management practices can be delivered. To this end this position will:

  • Establish a model of best practice for case management that is in line with NPY Women’s Council practice framework
  • Ensure that case management practices maintain cultural integrity
  • Oversee the monitoring and development of case management practices in the Youth Program.

KEY DUTIES

  • Work alongside the senior youth management team to provide mentoring and support the youth program staff to develop skills in case management.
  • Consult with the Youth Program Kulintja Palyaringkunytjaku “to get better thinking” project officer to develop client case plansfor clients who present with co-occurring substance abuse and mental health issues.
  • Develop a targeted approach to case management that involves individual and group work approaches to working with clients and their families.
  • Ongoing support, development and mentoring for Anangu Support Workers to build their individual and team capacity to support the delivery of case work services.
  • Promote the youth program through formalised referral pathways for clients entering and exiting the youth program.
  • Support senior management team to provide information and support to youth program staff for complex / high needs clients.
  • Provide staff with case work strategies that focus on targeting substance misuse, mental health issues, educational attainment, youth suicide and transition to employment or further training.
  • Communicare Database contact officer – compliance with data collection, report on trends re: case management and issues affecting young people.
  • Ensure Communicare data and reporting is done in a timely manner.
  • Prepare performance reports and other formal reports as requested by the Youth Program Manager.
  • Provide group learning sessions to build case management practices, knowledge and skill within the youth team.
  • Work alongside the Assistant Manager/s and Program Manager to ensure that all clients have a current case plan and support staff to develop these case plans
  • Coordinate and facilitate 6 weekly client case reviews.
  • Identify high risk clients and ensure close monitoring of case plans and support provided for staff.
  • Review new referrals in liaison with the youth program senior management team.
  • Make recommendations re: new referrals, existing clients and clients who are exiting the Youth Program.
  • Update client’s lists and provide regular reviews/updates of this list to the Manager and the Assistant Manager/s of the youth program.
  • Facilitate the development of risk assessments and risk management strategies.

SELECTION CRITERIA

Essential

  1. Qualifications and or extensive relevant experience in youth work, casework, community development or related human services work including supervision and support of staff.
  2. Understanding of current issues affecting young people in remote communities and the ability to provide leadership in relation to human service provision.
  3. Demonstrated ability to providesupervision, support and mentoring to staff ina fair and efficient manner.
  4. Experience in the development and implementation of projects and policy.
  5. A high level of expertise in written and verbal communication that will assist in dealing with a diverse range of people, including Anangu community members, NPYWCDirectors, staff and management,and government and other agencies.
  6. Above average competency in computer skills including with documents, email and the internet.
  7. Ability to operate in a cross-cultural environment, including working in partnership with staff and pursuing aims set by the NPYWC Directors.
  8. Ability and willingness to adhere to the policies and procedures of the organisation.
  9. Ability to work co-operatively within a large teamthat staffs a diverse range of skills and practitioners.
  10. Experience in,or capacity to work within, a community development framework.
  11. Ability to work with minimal supervision including being able to plan and use time effectively.
  12. Ability and willingness to undertake some bush travel as required.
  13. Current Australian manual driver’s license.
  14. Satisfactory criminal history check.
  15. Current first aid certificate or willingness to undertake training.
  16. Good level of health and fitness. Note: If so required by NPYWC at any time, you must undergo a satisfactory medical examination (including a pre-employment medical examination) for the purpose of determination whether you are able to perform the inherent requirements of your position. Any such medical examination will be at the employer’s cost, and the copies of any medical report will be provided to you. You must advise NPYWC of any illness, injury, disease, or any other matter relating to your health or physical fitness which may prevent you from performing your duties, or which may affect your ability to work safely.

Desirable

  1. Extensive case work and/or case management experience, particularly within the stated target group between 10 to 25 years of age.
  2. Experience in providing supervision to staff within the case work/management context, including delivery of training in this area.
  3. Experience working in Central Australia.
  4. Knowledge of NPYWC member communities.
  5. Knowledge of a WesternDesert language or willingness to learn.
  6. Experience in driving a 4WD or willing to undertake training.
