Section B - Supplies or Services and Prices/Costs

This solicitation is for a Firm, Fixed-Price, Lump Sum Offer. The solicitation requires bidding on all items. Failure to bid on all items will render the offer non-responsive. Fluor Federal Petroleum Operations, LLC (FFPO), at its discretion, may issue a single award, or reject any or all offers (See Section L).

The total lump sum offer price for all work herein shall be entered in the space provided in Section B-4 that follows, and on block 17, of the "Solicitation, Offer, and Award" (FFPOSF33 8/01/2014) contained in Section A Solicitation / Contract form, and blocks 18 through 20 of said document must be fully executed.

You must also complete the attached offer summary sheet and quantity sheet(s) (see instructions on pages B-3 & B-4).

SECTION B

SUPPLIES OR SERVICES AND PRICES / COSTS

B-1 SERVICES

The contractor shall provide all general construction services, to include labor, materials, equipment, tools, supplies, construction plant, supervision, transportation, and other services, necessary to complete the construction task as described in Section C, Statement of Work, in strict conformance with the contract clauses and provisions contained herein.

B-2CONTRACT TYPE

This is a firm, fixed-price contract for the total price of item Number 001, in Section B-4, below.

B-3ITEMS BEING ACQUIRED

In accordance with the requirements set forth in Section C, the Contractor shall provide the following:

Task BH-MM-1049, Replace 16 Inch Raw Water Surge Piping.

B-4PRICE

The Contractor shall deliver the following items in accordance with Sections C, F, and H and all other applicable terms and conditions of this contract.

BID SUMMARY SHEET INSTRUCTIONS

Col. 1CSI#: Construction Specifications Institute division number.

Col. 2Description: A general description of the CSI division number or subtask identified.

Col. 3Labor: The total transferred from col. 10 of the quantity sheet that depicts the labor cost to accomplish the CSI division being addressed.

Col. 4Material: The total transferred from col. 11 of the quantity sheet that depicts the material cost to accomplish the CSI division being addressed.

Col. 5Equipment: The total will be completed in item 11 below.

Col. 6Subcontract: The total transferred from col. 13 of the quantity sheet that depicts the subcontracted cost to accomplish the CSI division being addressed.

Col. 7Total: The sum of columns 3, 4, and 6, that depicts the total cost to accomplish the CSI division being addressed.

Item 8Benefits & Burdens: The cost of any payroll burdens and benefits applicable to crafts and supervision labor. Also, indicate percentage in the appropriate description column(s).

Item 9Supervision & Services: The total cost of all non-productive labor and services.

Item 10Construction Equipment: The total transferred from col 12 of the quantity sheet that depicts the cost of equipment required to accomplish this task.

Item 11Construction Plant: The total cost of field office and other field-support facilities.

Item 12Sm. Tools & Consumables: The total cost of required small tools and products that will be consumed to accomplish this task. Also, indicate percentage in item 12 description column.

Item 13Sales Tax: The total cost of sales tax applied to direct material, construction equipment, construction plant, and small tools & consumables. Also, indicate percentage in item 13 description column.

QUANTITY SHEET INSTRUCTIONS

Col. 1Description of Material, Operation, Equipment, or Services. Breakdown should be to sufficient detail to permit interpretation of all costs identified on the quantity sheet and should be grouped by CSI section.

Col. 2Quantity: Contractor's quantities required to perform the line item described.

Col. 3Units: Unit of measure required to the quantity described in col. 1.

Col. 4Unit Man-hours: Enter the unit man-hours to perform a single quantity described in col. 1.

Col. 5Total Man-hours: Enter the unit man-hours (col. 4) times the quantity unit (col. 2).

Col. 6Hourly Rate1: Enter the craft rate(s) used to accomplish the task described in col. 1.

Col. 7Material Unit Cost: Enter the unit cost of material, for a single quantity, required to accomplish the task described in col. 1.

Col. 8Equipment Unit Cost: Enter the unit cost of equipment, for a single quantity, required to accomplish the task described in col. 1. Equipment shall be listed as a separate section with appropriate daily, or weekly, or monthly rental rates.

Col. 9Subcontract Unit Cost: Enter the subcontract unit cost required, for a single quantity, to accomplish the task described in col. 1.

Col. 10Total Labor Amount: Enter the total of col. 6 times col. 5.

Col. 11Total Material Amount: Enter the total of col. 7 times col. 2.

Col. 12Total Equipment Amount: Enter the total of col. 8 times co. 2. with resulting total of col. 12 transferred to col. 14.

Col. 13Total Subcontract Amount: Enter the total of col. 9 times col. 2.

Col. 14Total Cost: Enter the sum of columns 10, 11, and 13. This will be the total cost of all operations to the left.

1 - Labor rates shall meet the minimum requirements of the Davis Bacon Act Wage Determination (see Section J). Should composite or crew rates be used, the contractor shall delineate the craft type, number of personnel per craft, and the proposed rate per craft. This information shall be included on the quantity sheet subsequent to the breakdown of the CSI section.

NOTE:

The following Summary Sheet and Quantity Sheet(s) contain the minimum breakdown required to be submitted with your offer. The line items contained therein may not necessarily include all tasks required to complete the entire statement of work requirements. Offerors are responsible for submitting quantities and summaries, which do include all elements necessary to perform and complete all statement of work requirements.

Offerors may give a more detailed breakdown, but you must give, as a minimum, the included breakdown.
