2017 YUTC Fundraising Commitment Fee:
Fundraising volunteers are crucial to the financial support of YUTC programs.
All members are required to participate in and support Club fund raising activities, such as track meet assistance when YUTC is the meet host, and other duties. A surcharge in the amount of $175.00, in the form of a cheque post dated for September 1st, 2017 is to be included with registration application at the time of membership registration.
All members are required to log a minimum of 15hours of volunteer time in assisting the club in fund raising activities or other duties. The fundraising cheque will returned/reimbursed upon completion of 15 hours of volunteer work for the club or by involvement in other specific areas as approved by the Board of Directors. The Fundraising Volunteer Commitment cheque will be deposited on September 1st, 2017and will be reimbursed upon completion of the 15hours of volunteer time. Members who choose not to participate or complete their volunteer commitment by the end of the last YUTC event will not have their volunteer commitment fee returned. A portion of the YUTC’s income is from the organization and coordination of track meets and other fundraising activities. High school athletes may also use this towards their school Volunteer Hours Requirements. A volunteer can be designated to fulfill volunteer requirements on behalf of the YUTC Member (i.e., parents, guardians, spouse, siblings, etc.).

IN CONSIDERATION of the acceptance of my application and the permission to participate as an entrant of membership, practice, competition, or any other event as part of theYork University Track & Field Clubduring the calendar year of 2017.

The Minor Track Association of Ontario
Athletics Ontario, Athletics Canada
and all other associations, sanctioning bodies and sponsoring companies, and all other agents, officials, servants, contractors, representatives, successors and assigns
OF AND FROM ANY AND ALL CLAIMS,
demands, damages, costs, expenses, actions and causes of actions, where in law or equity, in respect of death, injury, loss or damage my person or property, HOWEVER CAUSED arising or to arise by reason of said my participation in said, whether as a spectator, participant, competitor or otherwise; whether prior to, during or subsequent to any such event and NOTWITHSTANDING that some may have contributed to or occasioned by the negligence of any of the aforesaid.

2017 YUTC Code of Conduct
This Code of Conduct pertains to all members of the York University Track & Field Club (YUTC),including members of the Executive Committee, Coaches, Athletes and Parents or Legal Guardians of Members.
Each YUTCmember is required to complete this declaration on an annual basis at the time of registration with the Club. Members will be considered “Not-In-Good-Standing” until the Club Manager receives a completed document.
As a valuable member of the York University Track & Field Club (YUTC)…
I(we) do promise…
1.To understand and accept that I(we) are ambassadors to the sport of Athletics, that I (we) represent the York University Track & Field Club (YUTC) and I(we) will do nothing to bring discredit upon the Club, the YUTCCoaching Staff, other YUTCMembers, meet officials or myself;
2.To understand, accept and abide by the York University Track & Field Club (YUTC)Policies and By-Laws;
3.To accept the following Code of Conduct as a condition of retaining a “Membership-In-Good-Standing”;
4.I (we) will not possess, steal, destroy, vandalize, or in any way endanger the safety or property of others;
5.I (we) will not possess or use illegal drugs or controlled substances while engaged in YUTCevents, trips or competitions;
6.That when I(we) are participating in a YUTCevent, I(we) will:
- Comply with any team rules, including nightly curfews, established by YUTCExecutive Committee or the Coaching Staff;
- Attend all team functions, including meetings, practices, meals, competitions and any other function designed by the YUTCExecutive Committee or the Coaching Staff, unless specifically excused by one of the above;
- When unable to travel with parents or legal guardian, I(we) will travel with the team unless other specific arrangements are made known, and approved by, YUTCExecutive Committee and/or the Coaching Staff.
- Compete in the competition events chosen by the YUTCExecutive Committee and/or the Coaching Staff whenever possible;
- Comply with established uniform requirements;
- Act respectfully toward other non-YUTCteam members, coaches, officials, administrators, volunteers and fellow athletes;
I (we) understand that failure to comply with the above conditions of the Code will result in disciplinary action, which may include, but is not limited to the following (listed in order of severity):
- Termination of participation in non-competitive activities;
- Termination of participation in one or more competitive events;
- If traveling on an Club event, return home at my own or parental expense;
- Denial of future YUTCtravel assistance;
- Disqualification from future YUTCtravel teams;
- A change in my YUTCmembership status to “Not-In-Good-Standing”;
- Return of YUTCClub uniform;
- Refusal for Annual Membership Renewal;
- Expulsion from the University Track & Field Club (YUTC)and Refusal for Annual Membership Renewal.
I (we) further understand that the YUTCExecutive Committee may impose additional sanctions after a timely review.
I (we) understand that I (we) may appeal any disciplinary action in writing to the Executive Committee.
