Baylor Scott & White Athletic Physicals 2016-2017
Georgetown HS, Benold, and Forbes Athletes, Fine Arts,ROTC, and HOSA Students (7-12)
When: Saturday, May 21st, 2016 (Registration deadline May 6, 2016)
What:Each year,many medical professionalsvolunteer to help Georgetown ISD conduct athletic physicals at the Baylor Scott & White Hospital located at 300 University Blvd., Round Rock, TX 78665.
Time:8:00am (GHS, Benold, Forbes)BE ON TIME!!!!!
Although there are no time slots, the physical does not usually take more than 45 minutes to complete once they begin. When the last GHS, Benold, or Forbes student in line goes through the door, they will begin physicals for the next school. Those not in line, will not receive a physical.People typically begin lining up around 7:30am.
Cost:$20.00All proceeds benefit the GHS Athletic Training Program. Everyone must sign up and payPRIOR to attending.*If you are not signed up, you will not receive a physical, and we are unable to sign you up there*
Refunds: Once you have registered, there will be NO REFUNDS for any reason.
Required Items: 1. UIL Physical/Medical History Form (1 Sheet) The medical history section must be filled out completely including the parent/guardian and student signature. The physical section will be filled out by the physician.
3. Catastrophic Insurance Fee ($10 – Football, $5 – All Other Sports) This is a separate fee from the physical and will be collected by the trainers on the day of the physicals. We will accept cash or a check made out to GISD Athletics.
4. T-shirt, shorts and tennis shoes
Sign-up:Middle Schools: Take $20 to your Athletic Coordinator by May 6th, 2016.
GHS: Take $20 to the Athletic Training Room by May 6th, 2016.
*Cash/Checks will be accepted. Please make checks out to GHS Athletic Training.
Copies:If you would like a copy of your physical before the training room closes for the summer, it may be picked up from May 23rd – 27th in the GHS Training Room. This is especially important for those students who may be attending summer camps.

The remainder of the required forms needed to participate in the 2016-2017 school year can be submitted online by visiting georgetownisd.rankonesport.comany time after May 1, 2016. We highly encourage all parents to submit the online form as soon as possible after May 1, 2016. The updated UIL Medical/History Formwill also be available for download after May 1, 2016 on this website.
