Description of service / Rehabilitation of patients with long term conditions such as MS, MND, GBS, Head Injuries, stroke, Brain Tumours and neurological conditions.
Other areas of interest and related experience available / MDT that provides effective and caring services.
Snowdon @ Home, CNRT.
Work/Shift pattern / Early Shift- 07:30 to 15:30
Late Shift- 13:00 to 21:00
Night Shift- 20:45 to 07:45
Dress/Uniform code / Uniform to be worn
Staff room / Rest Room facilities / Various, explained on first day with team and discussed at induction
Library / IT Resources / Online Library services, UHS or at University

Prior recommended reading / websites / resources / Recommended visit Solent website as a minimum.
Common assessments / interventions / procedures / treatments / evaluations / Patient assessment, Risk assessment, Continence assessment and care, Nutritional assessment, Falls assessment, Wound Care, Personal Hygiene, Medication administrationand advice, Pressure area care, Long term condition management, advice and support for patients and their carers.
GPM- Goal Planning Meetings
MDT-Multi Disciplinary Team Meetings
CHC- Continuing Health Care.
Common conditions seen / Long term conditions, DOLS, Patients requiring re-housing, nursing home or residential home.
Specific areas of expertise/clinical skills which students can observe / learn from / Practice / Catheterisation, NG tube insertion, PEG and RIG feeds, Wound care, Continence management, Patients self care management, applying splints, medication management. Continuing health care referrals to District Nurse, GP. Transferring Patients to UHS when acutely ill. Writing transfer letters to hand over nursing care to Nursing Home, Hospitals and discharge summaries.
List of inter-professional learning opportunities available during placement / MDT meetings, Working alongside with specialist nurses, attending out- patient clinic, working with Snowdon @ Home.
