Statement of Assurance Regarding IDEA Funds
The Applicant, Specialized Treatment Center, hereby expressly assures to the State Superintendent of Education that it will comply with the requirements listed herein.
Accordingly, as a condition to the receipt of federal and state Special Education funds, ______, the Applicant, Specialized Treatment Center, hereby agrees to and must:
1. Comply with all State and Federal laws and regulations, and in particular, the Alabama Administrative Code, Special Education Services, Chapter 290-8-9;
2. Comply with procedural due process relating to applicable notices, timelines and safeguards in accordance with State and Federal laws and regulations;
3. Comply with the requirements regarding the qualifications of teachers and paraprofessionals;
4. Use the Chalkable Special Education Tracking System (SETS) program to complete required forms;
5. Complete requested and/or required reports in a timely manner;
6. Maintain all appropriate records and provide them to the Alabama State Department of Education (ALSDE) as requested or needed for fiscal audit or program evaluation;
7. Work with the Local Education Agency (LEA) (fiscal agent) to ensure that all students with disabilities participate in the State Assessment Program in the required grade levels;
8. Work with the LEA (fiscal agent) and a local high school to issue diplomas to all students with disabilities who meet diploma requirements;
9. Maintain and safeguard the confidentiality of personally-identifiable data and records pursuant to the Family Educational Rights and Privacy Act (FERPA);
10. Expend and manage funds in accordance with applicable State and Federal laws and regulations, and per the Specialized Treatment Center’s approved ALSDE budget.

This Statement of Assurance shall be signed by the designated representative and the original returned to Mrs. Billie Thompson, STC Point of Contact, Special Education Services, on or before August 1, 2017.
