I, ______Parent or guardian of: ______do hereby agree to waive any and all rights to bring a lawsuit or any type of legal Actions against the Montour Little Spartans, Youth Football Organizations, The Montour School District, Its coaches, employees and members of the Sports Booster Groups for any injuries or harm received by my child while participating in any of the Youth Football practices, games. Including but not limited to any of the intramural or Organized Sports Programs in the Montour School District. This Waiver also applies to any and all activities sponsored or sanctioned by the Montour School District.

CODE OF CONDUCT

As a Participant and Parent/Guardian, I agree to adhere to the MLSCode of Conduct:

1.  Conduct themselves in a respectable, cooperative and mannerly fashion including but not limited to:

2.  NO FIGHTING, ARGUING, INTIMIDATION, STEALING, INAPPROPRIATE LANGUAGE

3.  Proper use and care of equipment is required. Participant, parents or guardian will be responsible for any damage to equipment from careless use or intentional damage. Failure to return equipment will result in loss of invitation to banquet and also be held financially responsible for replacement of equipment.

4.  Attendance-Absenteeism or Tardiness without proper excuse will not be permitted. A participant that continuously misses or is late for practice will result in game suspension.

5.  You must pick up your child on time. The coaches are volunteering their time and should not have to wait for parents after practice.

6.  Parents/Guardians are required to work a minimum of two (2) volunteer shifts per participant. Which may include; field setup, concession stand, 50/50 sales, holding sticks, or field cleanup. A schedule will be published at the beginning of the season. If you’re unable to work your scheduled time, it is YOUR RESPONSIBILITY to find a replacement.

Any Violation of the Code of Conduct, MLS and/or PYFL By-laws, will be reviewed and action taken by the MLS Board of Directors. For the most up to date information visit our website @ www.montourlittlespartans.org
