DESMOND stands for Diabetes Education and Self-Management for Ongoing and Newly Diagnosed and is a NICE recognised national programme. The DESMOND (Core and Newly Diagnosed/Foundation Modules (ND/F))are part of an established education programme designed to give people with Type 2 diabetes a good start in managing their condition.

In the DESMOND (ND/F) education courses two trained Educators work together to facilitate group education for people with diabetes. Only registered healthcare professionals (e.g. diabetes nurses, dietitians) could take on this role.

As a DESMOND trained educator, the educator must follow a prescribed training programme as set down by the DESMOND National Programme.

Job summary

To work within the Milton KeynesDiabetes Care DESMOND Service,in an educational role.


To be trained as a DESMOND trainer:

  1. Observing a local DESMOND education course
  2. Attending 2 daysCore and Newly Diagnosed / Foundation module training organised by the DESMOND National Programme which may involve travel in order to attend
  3. Committing to a Quality Development assessment in year 2 and attending a study day in year 3
  4. Using the development work book and other resources to support ongoing development as a DESMOND Educator

Previously Trained DESMOND trainer

As apreviously trained DESMOND educator and you will need to follow the Quality Development Framework, commit to attending an update study day and, where necessary, complete a Quality Development assessment bythe DESMOND National Programme


  • Responsible for deliveringDESMONDcourses in education and support for self-management to groups of people with Type 2 diabetes from Practices across the Milton Keynes Primary Care Trust area.
  • Be committed to the DESMOND philosophy and principles.
  • Deliver theDESMOND Newly Diagnosed/Foundation Programme according to the formal Curriculum and using the designated resources as described in the Educator training
  • Assist people with Type 2 diabetes to reflect on their current thoughts and feelings about their diabetes and its impact on their lives, to actively consider the health risks they face and work towards setting realistic short-termbehaviouralgoals for themselves
  • Commit to a ‘zero hours’ contract
  • Maintain competency by delivering a minimum of 5DESMONDcourses a year.
  • To ensure patient confidentiality at all times.
  • To be prepared to study in own time (e.g. using development work book, curriculum and training materials and preparing for sessions).
  • Attend a DESMOND educator meeting quarterly
  • To participate in personal development and reflection through giving and receiving feedback on performance as an Educator.
  • To carry out self evaluation after delivery of every DESMOND education course.
  • Attend an Educators’ regional study day as part of quality development and ongoing

professional development once every three years

  • Undergo a Disclosure and Barring Service (DBS) check (previously CRB check).

  • Reporting potential risks identified.

In addition to maintaining continued education through attendance at any courses and/or study days necessary to ensure that professional development requirements are met, the post holder will participate in any training programme implemented by the Practice as part of this employment, such training to include:

  • Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work.
  • Providing certificates for copying of mandatory training courses undertaken.


The post holder will strive to maintain quality within the practice, and will:

  • Alert other team members to issues of Clinical Governance issues, quality and risk; participate in Significant Event Analysis reviews
  • Assess own performance and take accountability for own actions, either directly or under supervision;
  • Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the team’s performance;
  • Work effectively with individuals in other agencies to meet patients’ needs;
  • Effectively manage own time, workload and resources. The post holder will also contribute to the overall team-working of the Practice putting the needs of the Practice first.

The post holder will:-

  • Apply Practice policies, standards and guidance
  • Discuss with other members of the team how the policies, standards and guidelines will affect own work.
  • Participate in audit where appropriate.
  • Work with the partners and management team to achieve standards of quality, performance standards, budgets and targets without compromising levels of patient healthcare.
  • Contribute towards the development and implementation of new standards,

policies and procedures that are/will be required by GP practices now and in the future.
