JOB APPLICATIONS & SUPPORTING DOCUMENTS INFORMATION SHEET FOR OUTSIDE APPLICANTS

Please carefully read over all the instructions below before completing and submitting your application. Failure to provide all the required information may result in an “ineligible(incomplete)” rating and/or not qualifying for priority preference (i.e. SEP, OAV).

Applications & forms are available at our website, sill-www.army.mil/dcp. Applications can be turned in at the Job Information Center during our operating hours (Tuesdays or Thursdays from 11:30 am until 3:00 pm) or inserted in our drop-box located outside the main (south) entrance of building 1721.

Application and additional documents for all outside applicants (not current NAF employees):

·  Complete Applications (DA FORM 3433 or Resume and DA FORM 3433-1).

·  Complete Application Supplement: ADDITIONAL PROFESSIONAL/PERSONAL INFORMATION Form & CURRENT EMPLOYMENT STATUS Form.

·  Complete Background Survey (SF 181).

·  IF the vacancy announcement states that a college transcript is required, submit a copy of your transcript with the application.

A SEPARATE APPLICATION PACKET IS REQUIRED FOR EACH POSITION THAT YOU ARE APPLYING FOR.

ADDITIONAL DOCUMENTS REQUIRED FOR:

All former NAF EMPLOYEES (FNE) who are eligible for Reinstatement:

·  Submit a copy of your last DA FORM 3434 (NOTIFICATION OF PERSONNEL ACTION) Separation/Resignation, if the effective date was no more than three (3) years earlier than for REINSTATEMENT which allows you to be non-competitively referred to an activity (position must be the same series, the same grade/level or lower as previously employed) although it doesn’t guarantee selection of employment.

All applicants applying for Lifeguard positions:

·  Submit a copy of your LIFEGUARD CERTIFICATION (front and back of card) as required on the vacancy announcement

ADDITIONAL DOCUMENTS REQUIRED FOR:

All applicants applying for Child & Youth Program Assistant and other positions located at Child & Youth Services (Tincher Child Development Center, Youth Services, School-Age Services, and Youth Sports & Fitness Program :

·  Complete Application (DA FORM 3433-2).

·  IF the vacancy announcement states that a college transcript is required, submit a copy with the application.

All applicants who are applying for Spouse Employment Preference (SEP):

·  Marriage must have occurred prior to the PCS move to Fort Sill and dependent travel must have been authorized and stated as such on the PCS orders.

·  Complete FS Form 695b (NONAPPROPRIATED FUND APPLICATION FOR MILITARY SPOUSE PREFERENCE FORT SILL, OKLAHOMA).

·  Submit a copy of service member’s (sponsor’s) PCS orders and amendments

(if necessary).

·  Submit a copy of your marriage certificate, if applicant’s name does not appear on the PCS orders as being authorized to travel with service member.

All applicants who are prior Active Duty Military Members:

·  If you have served active duty in the U.S. Military, you must submit the most recent copy your DD FORM 214, MEMBER #4 COPY (annotated at the bottom right-hand corner) with your employment application.

All applicants who are current Active Duty Miltary Members:

·  Completed Request for permission to work off-duty form (FS FL23).
