Dear Novato Parents,
Welcome to a new year of Novato High School Athletics. We look forward to the 2015-2016 season. If your child is interested in participating in a sport this year, please follow the instructions below to complete the new Athletics Release Form.
Effective this school year the Athletic Release Form will be completed online through our partner company, FamilyID. FamilyID is a secure registration site that provides an easy, user-friendly means of registering for athletics. You will need to create a family profile just once during your child’s 4-year high school athletics participation. Once registered, the system will keep track of your information in your family profile. By saving your information future registrations will be simplified.
INFORMATION NEEDED TO REGISTER
It will be helpful to have the following information on hand when completing the online registration form.
· Health Insurance Information (Carrier and Policy Number)
· Doctor Information (Name and Phone Number)
· Emergency contact information
To register follow these steps:
Go to NovatoHightschool.org → Click REGISTER FOR ATHLETICS → Click REGISTER NOW
Once on the Novato High School FamilyID Page follow these instructions:
1. Click on Sign Up in the upper right hand corner of the screen
§ Enter your family name, email address and create a password. You will receive an email with a link to confirm your new account (if you don’t see the email, check your spam or junk mail).
§ Click on the link in your email confirmation.
§ You will use this account
2. After confirming your account you may now register for athletics.
§ You may need to search Novato in the “Find Programs” tab.
3. Once on the Novato Page at FamilyID select the program (season) for which you would like to register.
a. Enter the required information and sign the agreements, click continue.
b. You will be prompted to confirm your registration on the next page.
Finally, you will be required to submit a Physicians Clearance. (See below)
PHYSICIAN’S CLEARANCE FORM
v Each school year your child must be cleared by a physician in order to participate in high school sports.
v You can either upload this form on the FamilyID registration page by clicking on ‘Upload File’ found in the Participant Information or turn it into the Athletic Director before your child will be allowed to participate in any tryouts or practices.
v A completed physical examination is required dated after June 15, 2015.

Physical Education Waiver Exemption
All students in grade 9 are required (Education Code 51225) to take physical education (PE), and students must complete a second year of PE in either grade 10, 11 or 12. Students in grades 10, 11, or 12 may complete an Exemption Request Form, and if approved, may exempt a student from a semester of the second year PE requirement. In order to be eligible for a Physical Education Exemption, all criteria below must be met. No retroactive requests for exemption will be approved. Sports played in grade 9 do not count towards a physical education exemption.
To be eligible to receive an exemption in grades 10, 11 or 12, students must complete one year of Physical Education and satisfactorily pass at least five out of six fitness areas of the grade nine California Physical Fitness Test administered in grade 9 (Education Code 51241).
Ninth grade students are not eligible for the Physical Education Exemption. The semester of PE Exemption will be the semester the sport is played (Winter sports may be posted in either the Fall or Spring semester). If a student is requesting exemption from the entire second year of Physical Education, two seasons of a sport (any sport) must be played and the Physical Education Exemption Request Form must be completed and turned in on time. The same sport may be played two consecutive years to count towards the second year exemption.
To apply for a Physical Education Exemption, obtain a copy of the Physical Education Exemption Request Form from the Counseling Office. Forms must be signed by the coach and Athletic Director to verify that the student has attended practice, participated in training exercises and completed the entire season of the sport. Parent/guardian permission to apply for an exemption is also required. Forms must be completed by December 1st for Fall Sports, March 1st for Winter Sports and June 1st for Spring sports. Physical Education Exemption Request Forms submitted after the season’s deadline WILL NOT BE ACCEPTED. Any Senior who has not fulfilled the Physical Exemption Form requirements entering their Senior year will be enrolled in PE 2. Dropping PE2 after the drop deadline will result in a “W” on the student’s transcript.

Winter Sports 2015/16: Boys Basketball, Girls Basketball, Boys Soccer, Girls Soccer, Wrestling, Cheer & Dance – Basketball season.
Spring Sports 2016: Baseball, Softball, Co-ed Track & Field, Boys Tennis, Co-ed Swimming, Boys Volleyball, Girls Lacrosse, Boys Lacrosse, Boys Golf.
