Final Report: Patty Johnson Seed Grant

The intent of the one-year grant report is to assess how seed grant participants used the funds provided and reflect on theirexperiences with the Patty Johnson Seed Grant. Please use the questions within the report summary as a guide for assessing your project.

Please note the Patty Johnson Seed Grant funds are for approved projects only. Any funds remaining after approved projects are completed must be returned. Funds cannot be used for purposes outside the approved project per the grant agreement. For further questions on grant use, you may contact your New Leadership mentor for assistance.

Section #1: Summary of activities and outcomes of the Project

Please summarize your project, including key dates and how your project plans might have changed.

  1. The details of your project(i.e. steps in planning the project, the benefit or value added to a community as result of the project, resources used, contacts made).
  2. Describe any changes made to your original plan.
  3. Please provide details on any items left to complete on the project.
  4. Did you collaborate/work with anyone on the project within the community to implement the project?
  5. If so, how did you make these contacts?
  6. Was the communication relationship with your collaborator effective? (Please describe your communication with your collaborator and any challenges if any.)

Section #3: Critique of the NEW Leadership Patty Seed Grant Process
