PSAT Registration Form

REGISTRATION DATES: August 29-September 22

WHAT: The PSAT is a standardized, multiple-choice test that includes three sections: critical reading, mathematics, and writing. The test provides practice for the SAT and offers insight for advising and counseling students.

**Students who have not completed Math 1 (Algebra 1) should not register for the PSAT. A limited number of fee waivers are also available for students based on financial need.

WHO: The PSAT allows 11th grades students the opportunity to potentially qualify for a National Merit scholarship if they receive a very high score (generally the top 1%). For this reason, WCPSS is offering the test free of charge to those students with a 4.5625 GPA or higher. All other students may take the PSAT/NMSQT for a fee of $15.

Payment: $15 to MCHS Online Payment system by Sept 22, 2016 (located under Parent Tab on the MCHS website)

HOW DO STUDENTS REGISTER:

1.  Submit payment to MCHS Online payment system by September 22, 2016.

2.  Submit the registration form to Student Services by September 22.

ACCOMMODATIONS: You should fill out the following section of this form only if you have accommodations (extended time, separate setting, etc.) approved through College Board OR if you intend to apply for accommodations through College Board. Students should have an existing IEP/504 to apply for accommodations.

**Students with an IEP/504 do not automatically qualify for SSD accommodations through College Board. It is the sole discretion of the College Board to approve these accommodations and not MCHS or WCPSS.**

______Apply for Accommodations. I have an IEP or 504 and am eligible to apply for College Board accommodations. I would like MCHS to apply for accommodations on my behalf. I understand that the deadline for accommodations is August 30, 2016. Homeschooled students must apply for accommodations directly with College Board and must provide MCHS with College Board documentation of approved accommodations no later than October 7.
