2014 Mathematics Instructional Materials Adoption (K–8)
Instructional Materials Submission List – Attachment D
The Instructional Materials Submission List is the list of materials that you will be submitting for each program.
Excel File Specifications
· The file must be a Microsoft Excel file. Header rows are optional and will be ignored. Headers cannot exceed 10 rows.
· The file must have 13 columns for fields #1 through #13 described below.
· When a field is optional, it can be left blank.
· Each program will be saved on a separate excel form.
· Excel files must be sent to with the subject line reading, ‘MATERIALS SUBMISSION BY [Publisher Name Here]’ no later than
May 15, 2013 at 5:00PM PST.
Program Descriptions
· Publisher: the name of the publisher publishing the program.
· Series: the title of the series for the program submitted.
· Author: the author(s) of the program submitted.
· Program Type: Choose one of the following:
o Basic grade level programs (K-8)
o Algebra 1
o [Integrated] Mathematics I
Field Descriptions
#1: ITEM / TITLE
Item is required. (i.e. title name) If item includes multiple titles, provide the title name and ISBN for all titles. Item cannot exceed 3000 characters.
#2: ISBN OR UNIQUE IDENTIFIER
An ISBN or other unique identifier is required. Hyphens will be ignored. If an ISBN is used, it must be 10 or 13 characters excluding hyphens.
#3: ALTERNATE ISBN
Alternate ISBN is optional. Alternate ISBN must be 13 numeric digits.
#4: LANGUAGE CODE
Language code is required. The language code must be one of the following, e.g. "00" for English.

#5: COPYRIGHT YEAR
Copyright year is required. It must be a 4 digit year.
#6: GRADE LEVEL FROM
Grade Level From is required. It must be one of the following, e.g. "8" for Grade 8.
· K - Kindergarten
· 1 - Grade 1
· 2 - Grade 2
· 3 - Grade 3
· 4 - Grade 4
· 5 - Grade 5
· 6 - Grade 6
· 7 - Grade 7
· 8 - Grade 8
#7: GRADE LEVEL TO
Grade Level To is optional if the codes are the same as those for field #6.
#8: NUMBER OF PAGES
Number of pages is required. Include front matter, body, and back matter.

Alternate format is optional. A blank will be the same as a "N". It must be one of the following.
