TERMS AND CONDITIONS OF SERVICE

The terms and conditions applicable to this post are those of all NHS Employees.

1. Superannuation

You have the option to join the NHS Superannuation Scheme, to participate in the State Earnings Related Pension Scheme or to take out a Personal Pension.

Employee’s contributions to the NHS Scheme amount to 5.2% to 14.7% of salary (depending on rate of Pensionable Pay) and the employers’ contribution equates to 13.5% of salary. Employees in the NHS Scheme are “Contracted-out” of the State Earnings Related Pension Scheme and pay a lower rate of National Insurance contributions. Employees who choose to participate in the State Earnings Related Pension Scheme pay the higher rate of National Insurance contribution. A Stakeholder Pension is also available.

2. JOB PURPOSE

To deliver high-quality evidence-based clinical care in the hospital setting, working autonomously as part of a multi-disciplinary team.
To be involved in the further development of new systems of working, particularly in relation to new and enhanced roles.
To participate in teaching, audit and research.
The post holder will be supervised by a named Orthopaedic Consultant.
3. DIMENSIONS
Physician Assistants are highly trained professionals who can provide a wide range of healthcare services to patients, ranging from taking a history, performing a physical examination, requesting and interpreting diagnostic tests to deciding appropriate clinical management and providing advice and drugs. They work under the supervision of a doctor and are part of the wider healthcare team.
Appointees to this post will join the existing multidisciplinary team within the orthopaedic department. They will be responsible for delivering high-quality inpatient and outpatient care as well as participating in teaching, research and audit, and actively developing the role of the Physician Assistant within the department.
The post holder will be expected to work closely with Medical Consultants, General Medical Practitioners (GPs), Nurses and Allied Health Professionals, Managers and Administrative and Clerical staff.

4. ORGANISATIONAL POSITION

5.  ROLE OF THE DEPARTMENT
The Golden Jubilee National Hospital is situated in extensive grounds on the banks of the River Clyde and has excellent national communication links. Glasgow International Airport is only 15 minutes away by car and Glasgow City Centre (with internationally renowned shopping and dining) is less than half an hour. Scotland’s historic capital, Edinburgh is within 90 minutes’ travel and Loch Lomond, one of the world’s true scenic destinations is only a short drive. Dalmuir Railway station (some 15 minutes walk from the hospital) has very frequent services to the centre of Glasgow and thence throughout the United Kingdom. The hospital is linked in the same site to the Beardmore four-star hotel, which offers special rates for staff and relatives.
The orthopaedic department consists of fourteen full time Consultants. One consultant has a special interest in foot and ankle surgery and dedicates around two-thirds of their practice to this field. The remaining consultants have special interests in hip and knee replacement although some general orthopaedic practice is also carried out. Five orthopaedic theatres are available for major joint surgery from Monday to Friday. The department has access to 64 dedicated elective orthopaedic beds and there is a separate day-case facility. Each patient occupies a single room. A new orthopaedic outpatient department and office block opened in April 2011 and the hospital has excellent rehabilitation and radiological services.
The department has seen significant expansion in recent years. It now undertakes some 3000 hip and knee replacements (primary and revision) annually making it one of the biggest arthroplasty centres in the United Kingdom. An additional 700 elective non-arthroplasty procedures (anterior cruciate ligament reconstructions, hand and foot surgery) are also performed per year. The hospital is somewhat unique in Scotland in taking patients from a very wide geographical area, either as tertiary referrals or to help local health Boards meet their waiting time commitments.
The orthopaedic unit appointed its first Physician Assistant in 2011. The department is enthusiastic about the role of Physician Assistants, and wishes to develop the role by converting a number of posts currently held by junior doctors to additional Physician Assistant posts. Physician Assistants will assist consultants and clinical fellows in theatre, and will treat orthopaedic outpatients and ward patients (under consultant supervision). The main duties are in elective orthopaedics, and there is at present no “on-call” commitment. The majority of the working week is consequently within weekday hours. Mentoring will be undertaken by orthopaedic consultants and training will also be provided to allow successful candidates to take on the full range of duties within the job.
The department has a history of innovation and has very active research and audit programmes overseen by a Research Co-ordinator. A novel enhanced recovery programme (“Caledonian” technique) was developed within the hospital and has led to a significant reduction in the hospital stay. This technique has been widely adopted throughout the country, and pioneering work has also been done in fields such as wound care and computer navigation in joint replacement surgery.
As well as a growing and dynamic orthopaedic department, the Golden Jubilee National Hospital has large Interventional Cardiology and Cardiothoracic Surgery departments. Plastic Surgery, General Surgery, Endoscopy, Ophthalmology and Bariatric Surgery services are also offered.

6. MAIN DUTIES

The department is looking for trained Physician Assistants (PAs) who are able to function flexibly to deliver orthopaedic care. The main responsibilities of the post are mainly (but not exclusively):
Theatre
The post-holder will assist consultants and clinical fellows in undertaking operations, and (with suitable experience) undertake wound closure and minor procedures. Theatre commitments are variable, but two or three days per week in theatre is typical.
Clinic
PAs and Clinical Fellows assist consultants in new and return patient clinics, seeing patients and formulating a management plan in conjunction with the consultant. The post-holders will use their skills in history taking and physical examination. They will order and interpret investigations such as x-rays, blood tests and scans. They will spend time discussing treatment options with patients and answering questions as part of the consenting process.
Ward
PAs will undertake pre-operative and post-operative ward rounds, either independently or with consultants. They will assist the Resident Medical Officers with routine ward work and in the management of post-operative complications. The keeping of accurate medical records is essential.
Administrative
The posts are primarily practical, but include some administrative work such as the preparing of discharge and clinic letters and collation of audit data.
Teaching, Audit and Research
The department has an active post-graduate teaching programme, currently comprising two afternoons a month and including morbidity and mortality meetings as well as topics of general orthopaedic interest. There will be opportunities for post-holders to participate actively in these sessions. Interested candidates will also be able to participate in the department’s dynamic research and audit programme.
