Request Forms

Forms SS-16, SS-16a, and the SS-16a Continuation Page are forms that are generated from our database and therefore cannot be made available online. However, Master Commissioners already registered with our office can request additional forms by e-mail. Please include Name, County, Office Code and number of forms needed in the e-mail. (The Office Code is the four digit number the Social Security Coverage and Reporting Branch assign to each Master Commissioner.) Upon receipt of the request, the forms will be mailed within two business days.

Note: Newly appointed Master Commissioners will need to complete the registration process with our office before forms can be sent.

Form SS-16

Form SS-16 is used to report Master Commissioners and their deputy’s salaries, wages, and fees for social security and medicare (FICA) tax purposes.

Form SS-16a

Form SS-16a is used to report Master Commissioners and their deputy’s salaries, wages, and fees for social security and medicare (FICA), federal income tax and state income tax purposes.

(You must participate in the voluntary withholding of state and federal income taxes to receive Form SS-16a.)

SS-16a Continuation Page

The SS-16a continuation page is to be used in conjunction with Forms SS-16a when reporting local wages and taxes and/or deferred compensationfrom performance of service as Master Commissioner. The continuation page does not eliminate the need for Form SS-16a, you would need to fill out and send us both the Form SS-16a and the SS-16a continuation page when reporting. If you are not reporting any local wages and taxes or deferred compensation then the continuation page is not needed. Only Master Commissioners participating in voluntary withholding are eligible to report local wages and taxes and/or deferred compensation.

Note: Reporting local wages and taxes and/or deferred compensation to our office is for the purpose of possibly eliminating the need for additional Forms W-2 for these amounts. The Master Commissioner cannot send those payments to the Social Security Coverage and Reporting Branch. You will continue to make those payments how you have in the past.
