As you may be aware, the new South Glasgow University Hospital and new Royal Hospital for Sick Children are due to open on the current Southern site early in 2015.

With this in mind, please note that positions based within the Victoria Infirmary, Mansionhouse Unit, the Western Infirmary and the current Royal Hospital for Sick Children at Yorkhill will change location and move to the new hospitals.

Gartnavel General Hospital and Glasgow Royal Infirmary will also have some services affected by moves to the new Hospitals.

These changes mean your base may change after joining us and you will be informed as soon as possible prior to any change of base.

SUMMARY INFORMATION RELATING TO THIS POSITION

Post: SENIOR CLINICAL FELLOW paediatric cardiology with an interEst in cardiac catheterization

Base: ROYAL HOSPITAL FOR SICK CHILDREN

This cardiology fellow post with an interest in cardiac catheterization will based at the Royal Hospital for Sick Children, Glasgow. The position is for a period of up to one year but applicants seeking shorter terms will be welcomed. You will already have a good theoretical knowledge of paediatric cardiac catheterization and be able to undertake simple diagnostic and interventional cases with consultant supervision, and will be responsible for preparation of catheter reports and their presentation at the weekly MDT meeting. You will also undertake outpatient clinics as required and be responsible for occasional undergraduate clinical teaching. There may be scope for the successful candidate to join the after-hours rota for cardiology and to participate in clinical audit/research.

To practice medicine in the UK you need to hold both GMC registration and a licence to practice.

The Hospital

The Royal Hospital for Sick Children, Glasgow, is the largest paediatric teaching hospital in Scotland. It provides care, not only for children resident within Greater Glasgow, but is also a tertiary referral centre for children from the West of Scotland and, in some sub-specialties, from the whole of Scotland.

There is a complement of 348 beds, which includes 24 neonatal surgery cots. All paediatric medical and surgical subspecialties are represented, including general medical paediatrics, cardiology, neonatology, neurology, nephrology, respiratory medicine, endocrinology, gastroenterology, immunology and infectious diseases, dermatology, haematology/oncology, audiology, ophthalmology, ENT surgery, orthopaedics and general paediatric and neonatal surgery. The hospital provides the Scottish national ECMO service for paediatric and neonatal patients. It is the centre for all paediatric cardiac surgery and the majority of the Paediatric Intensive Care for Scotland

The hospital provides the major Undergraduate Paediatric Teaching facility for the University of Glasgow and accommodates all or part of the University Departments of Child Health, Child and Adolescent Psychiatry, Medical Genetics, Human Nutrition, Paediatric Pathology, Paediatric Biochemistry and Paediatric Surgery.

The Post

The clinical fellow post is available from the end of June, beginning of July. It is based in the Paediatric Cardiology service for up to a 12 month period with emphasis on gaining higher skills in cardiac catheterization and intervention.

Relevant Departments

Paediatric Cardiology

The hospital provides the national paediatric cardiac centre for Scotland, serving a population of 5.5 million and undertaking all types of cardiac surgery and catheter intervention with the exception of cardiac transplantation. Consequently the management of cardiac patients forms a significant percentage of PICU workload and experience.

Fellows are involved in the preoperative preparation of patients undergoing elective cardiac procedures as well as the ward based postoperative management. They contribute to the daily PICU cardiac ward round, learning more about the ward/PICU interactions. There is also regular involvement in the assessment of infants referred into the neonatal unit with cardiac conditions. There are regular out-patient clinic commitments and also the opportunity to learn echocardiography, through a weekly dedicated echo session under the supervision of a senior cardiac physiologist, and attendance at our 4 day paediatric echocardiography course. There is a fully functioning Cardiac Catheterization Laboratory with diagnostic and interventional procedures undertaken on a regular basis. There will be a 1:5 out-of-hours commitment responsible for first line management of the Paediatric Cardiology patients between 09.00 and 21.30 (after 21:30 this cover is provided by the Hospital at Night team).

Neonatal Intensive Care

Neonatal care at Yorkhill is part centrally funded by NSD with several national services provided on site. This includes neonatal cardiac surgery, ECMO, neonatal airway surgery, Vein of Galen treatment (UK/European service) and Congenital Diaphragmatic Hernia service. The NICU (12 ITU cots) is jointly managed by the neonatal consultants and the paediatric surgeons. Neonatal ECMO for Scotland and the North of Ireland is based in the unit. Over 200 newborns per year are currently transferred to Yorkhill because of the specialist services offered on site: one third are cardiology patients, one third are transfers of pregnancies affected by a congenital anomaly and the remainder are newborns requiring the involvement of the other paediatric specialists on site. Neonatal cardiology admissions for the whole of Scotland are admitted to this unit for further assessment and management in conjunction with the Cardiology service.

ROTAS

There is no out-of-hours work involved with this post. However from time to time there may be the opportunity to join the evening (1700-2130) rota as locum first on-call for the cardiology ward.

TEACHING

There are occasional undergraduate teaching commitments

STUDY AND TRAINING

Posts are recognised for postgraduate training. Study Leave within United Kingdom will be in accordance with the Terms and Conditions of Service and application should be submitted to the relevant Clinical Director. The Postgraduate Tutor for the RHSC complex is Mr R Carachi. There is good medical library within RHSC and extensive library facilities in Glasgow.

POSTGRADUATE MEDICAL EDUCATION COURSE AVAILABLE LOCALLY

There are frequent lectures within the University of Glasgow and the Royal College of Physicians and Surgeons of Glasgow relevant to postgraduate medical education. Special courses and seminars are arranged within the hospital from time to time. Formal in-post training within the hospital is arranged by the Medical division, by the postgraduate Tutor and by the Medical Staff Association.

RESEARCH OPPORTUNITIES

There are opportunities for clinical research and include laboratory and animal facilities if required. Any research protocol must first be agreed by the Hospital Ethical Committee before it can be instituted.

DETAILS OF ARRANGEMENTS FOR APPLICANTS TO VISIT HOSPITAL(S)

Shortlisted candidates may make arrangements to visit relevant hospitals. If candidates on their own initiative have visited prior to short-listing, they will only be allowed expenses for that prior visit if they are subsequently short-listed. When it is thought that there will be difficulty in filling the post, the Director of Human Resources has the authority to approve a second visit.

MEDICAL NEGLIGENCE

In terms of NHS Circular 1989(PSC)32 dealing with Medical Negligence, the Trust does not require you to subscribe to a Medical Defence Organisation. The Trust will indemnify you under the Crown Indemnity Scheme for all work carried out for the Trust. It may however be in your interest to subscribe to a defence body in order to ensure that you are covered for any work that does not fall within the scope of the indemnity scheme.

HOURS OF WORK

In terms of paragraph 111b of the Terms and Conditions of Service, you shall not undertake locum medical or dental work for any other employer where such work would cause your average contracted hours to breach the controls as set out in paragraph 20, i.e. on-call rota - 72 hours/week, partial shifts - 64 hours/week, full shifts - 56 hours/week.

TERMS AND CONDITIONS OF SERVICE

The conditions of service are those laid down and amended from time to time by the Hospital and Medical & Dental Whitley Council.

£ 30,605 - £ 48,123 per annum (pro rata)
New Entrants to the NHS will normally commence on the minimum point of the salary scale, (dependent on qualifications and experience). Salary is paid monthly by Bank Credit Transfer.
HOURS OF DUTY / Full Time 40.00
SUPERANNUATION / You have the option to join the NHS Superannuation Scheme, to participate in the State Earnings Related Pension Scheme or to take out a Personal Pension. Employee’s contributions to the NHS Scheme are Tiered based on your earnings and the employers contribution equates to 13.5 % of salary. Employees in the NHS Scheme are “Contracted-out” of the State Earnings Related Pension Scheme and pay a lower rate of National Insurance contributions. Employees who choose to participate in the State Earnings Related Pension Scheme pay the higher rate of National Insurance contribution. A Stakeholder Pension is also available. A Personal Pension is a private arrangement agreed with the pension provider that will be an organisation such as a Bank, Building Society or Insurance Company.
REMOVAL EXPENSES / Assistance with removal and associated expenses may be given and would be discussed and agreed prior to appointment.
EXPENSES OF CANDIDATES FOR APPOINTMENT / Candidates who are requested to attend an interview will be given assistance with appropriate travelling expenses. Re-imbursement shall not normally be made to employees who withdraw their application or refuse an offer of appointment.
TOBACCO POLICY / NHS Greater Glasgow and Clyde operate a No Smoking Policy in all premises and grounds.
DISCLOSURE SCOTLAND / This post is considered to be in the category of “Regulated Work” and therefore requires a Disclosure Scotland Protection of Vulnerable Groups Scheme (PVG) Membership which currently costs £59.00. The cost of the PVG Membership will be initially paid by NHS Greater Glasgow and Clyde and will required to be repaid through a payroll deduction mandate from the successful candidate’s first salary.
