HOW TO REGISTER WITH THE PRACTICE
Prospective patients or their representative must attend the surgery to register.
There are two forms to be completed, they may be completed at the surgery or taken away and returned in person to be checked. If you have access to the internet these forms can be downloaded and completed at home then brought to the surgery, they cannot be emailed to us as we require a signature.
*Registration form – This is a contract between you and the practice, signing it allows us to obtain your previous records and claim NHS payments for your care.
*Health questionnaire – This gives us some back ground information while we send for your records.
If you take regular medication, or have a complex past history you may be asked to attend for a health assessment or prescription review.
Non-British nationals will be required to provide proof that they entered the country with the correct visa and are therefore eligible for NHS treatment.
Once the registration form is completed and returned you are registered with the practice and eligible for treatment here from that time.

Receptionists
The receptionists are the first people you will meet at the practice. They will help you with appointments, prescription requests and general queries and liaise with other members of the practice team on your behalf. Any information they receive is treated in the strictest confidence. They follow guidelines laid down by the Doctors and treat everyone equally.

Did you know that you can get free, fast and expert health advice at any pharmacy in Sefton?
“Care at the Chemist” scheme is the quickest and easiest way to get advice and treatment from a health professional for a wide range of everyday illnesses and ailments – without even setting foot in your doctor’s surgery.
So, if you have a common or minor health problem, Care at the Chemist is for you. You don’t need an appointment and you’ll find a chemist close to home, with many open early until late.
Medicines are free for anyone who does not pay for their prescriptions – all you need is proof of exemption. Those who do pay will be charged the current prescription charge. If the medicine costs less to buy over the counter than the prescription charge, you will pay the lower rate.

Missed Appointments
Patients who do not attend their appointments are a drain on surgery time and create problems for us all. We therefore have a policy to deal with this problem.
- First appointment missed you will receive a letter from the practice.
- Second appointment missed you will be informed that persistent non-attendance may result in your removal from the practice list.
- Further missed appointments will result in your removal from the list.
If you are unable to keep your appointment please notify the surgery as soon as possible.
RIGHTS & RESPONSIBILITIES
Patients can expect:
To be treated with respect no matter what their age, race, gender or religion.
An urgent appointment with an unspecified doctor on the day of the request.
A routine appointment with the first available doctor within 2 working days. Not to be kept
waiting longer than necessary and to be informed of the reason if the wait is longer than 15
minutes after their appointment time.
Repeat prescriptions will be ready within 2 working days. This applies to medication on the
repeat slip only.
Patients should:
Treat all doctors, nurses and other staff with the same courtesy and respect they expect for
themselves.
Attend on time for appointments or inform us as soon as possible if you cannot keep or do not
need your appointment.
Remember that an appointment is for 1 person only.
Only request a visit for patients who are so ill they cannot be brought to the surgery.
Telephone requests for visits before 10 am. Lack of transport is not a reason for a visit.
Not telephone the surgery to order repeat prescriptions.
Each patient is responsible for their own health and should take every step to prevent illness
with the help and advice of health professionals.
PRIVACY

CCTV
The practice operates CCTV both in the practice and the car park to ensure the security and safety of our patients and staff.

The pharmacists run clinics to review medication and carry out health checks and are available to answer your queries about the medicines you take.

Our Health Care Assistants carry out BP checks and can provide advice on giving up smoking.
Medical students, Foundation Doctors and Specialist Trainees:
St Marks is a well-established training practice and we will usually have junior doctors working with us. Specialist trainees (formerly known as GP Registrars) and Foundation doctors are fully qualified doctors with some hospital experience who are with us for a period of four, six or twelve months.

District Nurses
Our district nurse team provide nursing services to housebound patients. Patients can be referred to the district nurses by the doctors at the practice or by the hospital discharge planning team.
Midwives
Midwives care for women during pregnancy and for the first 10 days after their baby is born.
They follow a protocol for shared care; copies of this are available in the practice.
The midwives hold ante-natal clinics on Monday morning and Friday afternoon by appointment
Practice Nurses
Babies and children by appointment for development checks and immunisations.
Dietitians
Community dietitians hold regular clinics at the practice. You may be referred to them by your GP or one of the Practice Nurses.
ORDERING REPEAT PRESCRIPTIONS
Please retain the repeat slip attached to your prescription and use it to re order your prescription.
Allow 48 hrs (two full working days) for the prescriptions to be ready for collection to ensure you have the least inconvenience.
Prescription requests may be made Online, by e-mail, by post to the practice or made through the pharmacy of your choice.
Prescriptions cannot be requested by telephone as this blocks the phone lines.
Stamp addressed envelopes are required if patients wish the prescription to be returned to their home address.
Most pharmacies will collect your prescription for you if you have difficulty getting to the practice; speak to your pharmacist to arrange this.
CHAPERONES
You are entitled to have a chaperone present during a physical examination. Please ask at the time of your appointment.
CONFIDENTIALITY
The practice complies with Data Protection and Access to Medical Records legislation. Identifiable information about you will be shared with others:
To provide further medical treatment for e.g. from district nurses/hospital services.
To help you get other services e.g. social work department (requires your consent).
When we have a duty to others e.g. in child protection cases, anonymous patient information
will also be used at local and national level to help the Health Board and Government plan
services e.g. for diabetic care.
If you do not wish anonymous information about you to be used in such a way, please let us know. Reception and administration staff require access to your medical records in order to do their jobs. These members of staff are bound by the same rules of confidentiality as the medical staff.
OUT OF HOURS EMERGENCY SERVICE
After 6.30 pm weekdays, at weekends and on Public Holidays our Out of Hours service is
provided by Go to Doc and they can be contacted on 0845 070 4422.
This service is for urgent problems only. Please do not abuse this service by ringing about
complaints that could wait until the surgery opens.
The majority of patients after 6.30pm will be seen at the GP on call centre at Southport and
District General Hospital.
Please ring your normal practice number out of hours and listen to the message, have a pen
and paper ready to take down the out of hours emergency number.

COMMENTS & COMPLAINTS.
The practice is interested in any comments you wish to make about our services. Excellence in patient care is what we aim for and we will always consider all comments and feedback from patients in helping us to achieve this and make improvements where necessary.
We have a box in the waiting room where you can post comments (anonymously if you wish).
We follow the guidelines for all primary care teams in having a complaints procedure.

The surgery has a small car park.
Some space must be reserved for doctors and other essential staff.
Please use public transport or walk to the surgery if you can.
We request that the available parking spaces are reserved for the disabled and acutely ill
patients only.

PUBLIC TRANSPORT
The surgery is on the route of the number 43 and 45 bus, the number 46 bus stops on Manchester Road near the junction with Marlborough Gardens which is a short walk away.
THE PRACTICE AREA
Northern Boundary
Marshside Road from its junction with the Coastal Road/Marine Drive South Eastwards to the junction with Preston New Road then North Eastwards along the East side of Preston New Road to the Plough Roundabout and then Eastwards along Water Lane to the West Lancashire CC Boundary.
Eastern Boundary
From Water Lane in the North Southwards along the West Lancs CC Boundary to Southport Road then North Westwards to the Kew Roundabout then South Westwards along Town Lane and Benthams Way to the southern end of Kew Road.
Southern Boundary
Southern end of Kew Road from the West Lancs CC boundary North Westwards to its junction with Eastbourne Road then South along Eastbourne Road to its junction with Liverpool Road and then North Westwards along Liverpool Road and Weld Road to the Coastal Road/Marine Drive.
Western Boundary

HOW TO MAKE AN APPOINTMENT
We run an appointment system - appointments may be pre-booked up to 2 weeks in advance.
The GPs have 15 minutes per appointment, if you have several problems and think you need
more time, please let the receptionist know.
Each appointment is for 1 person only.
If you wish to see a particular doctor you may need to wait longer especially if that doctor only
works part- time.
If you need to be seen urgently please ring as early as possible in the day.
HOME VISITS
If you think you need a home visit please ring in the request before 10am.
Home visits are reserved for patients who are truly housebound or so incapacitated that they
cannot be brought to surgery.
Children, in almost all cases, can be brought to surgery and seen in the best conditions for
examination. This is safer for your child as urgent problems can be referred quickly to hospital.
Unfortunately the cost of travelling to the surgery cannot be considered. Please make sure you
have a system so that you can access travel either byfamily, friend or taxi for emergencies.
GPs will consider all requests for home visits and they will decide, on the information provided,
whether a Home Visit is necessary.
The majority of patients after 6.30pm will be seen at the GP on call centre at Southport and
District General Hospital.
Please ring your normal practice number out of hours and listen to the message, have a pen
and paper ready to take down the out of hoursemergency number.
HOW TO SPEAK TO THE DOCTOR OR NURSE ON THE TELEPHONE
Telephone consultations can be made – you book these up to two weeks in advance.
USE OF 999
This important emergency service is for real emergencies.
It activates a huge resource and if misused could mean life or death to someone else.
Severe chest pain, sudden disabling breathlessness, collapse and severe injury are the proper reasons for contacting this number
If you are in doubt please phone the surgery or ring NHS 111
NHS 111 will give you advice on any medical problem if required.

We do have a number of pre-bookable appointments for your convenience . The easiest way for you to make an appointment is to sign up to Patient Access where you can book an appointment “on-line”, for those of you who have a smart phone, you can download the APP for this. Please ask at Reception for further information on Patient Access.

FREEDOM OF INFORMATION ACT
The Information Commissioners Office has published a Model Publication Scheme that the Practice has adopted.
VIOLENCE AND AGGRESSIVE BEHAVIOUR
The Practice will not tolerate anti-social or aggressive behaviour towards a doctor, a member of staff, another patient, or who damages property and a request for the removal from the practice list of any patient acting in such a manner will be requested and the CCG will allocate them to another practice.
Aggressive behaviour is considered to be any personal, abusive and/or aggressive comments, cursing and/or swearing, physical contact and/or aggressive gestures. All instances of actual physical abuse on any doctor or member of staff, by a patient or their relatives will be reported to the police as an assault.
