Easy Steps to Requesting a Leave of Absence (LOA)
1. Talk to your manager as soon as you discover you may need to take a LOA; LOA requests are required for time off from work in excess of 1 week for reasons other than pre-scheduled departmental PPL requests.
2. Access the necessary paperwork on the Human Resources (HR) SharePoint site or, if you are outside the facility, the Lourdes’ website, www.lourdesmed.org/employees. The paperwork is separated by the different types of leave. If you cannot access the paperwork on your own, ask your manager to access and print this paperwork for you.
3. Complete the Associate Request for a LOA form. Be sure that you provide dates and that you and your manager sign and date this form. Fax or mail the form to HR at the number / address on the form or ask your manager to do this for you.
4. Take the Certification of Healthcare Provider form to the treating health care provider to be completed. Once complete, send this form directly to HR at the address provided. DO NOT GIVE THE CERTIFICATION OF HEALTH CARE PROVIDER FORM OR ANY OTHER MEDICAL DOCUMENTATION TO YOUR SUPERVISOR.
5. You will receive notification from HR approving, denying, or requesting additional information for your LOA. This approval is for job-protection only. NJ State Income Benefits are a separate process. The forms are available with the LOA forms. They should be submitted directly to the State according to the instructions on each form.

· Retain a copy of all information and forms for your records.
· You are your supervisor must work together to schedule your actual return to work. You should contact your supervisor as soon as possible with any changes to your tentative return to work date.
