The following questions measure behavioral health aspects, which may be indirectly related to cardiovascular disease.

Occupational stress awareness consultation is now included in the NFPA 1582 7.7.23 (7.7.23.1-2)(2017). These questionnaires help to provide this awareness education to you. In addition, we have added sleep disturbance questionnaires to the Health History Questionnaire. Sleep disturbance screening is now include in the NFPA 1582 7.7.21 (7.7.21.1-2)(2017).
