I, , attest that, in accordance with Section 142.006,Texas Human Resources Code,and Texas Administrative Code Title 37, Chapter 341, Subchapter H, all eligibility requirements have been met, completed, and attained,including the successful completion of the Texas Commission on Law Enforcement (TCOLE) firearms training program and TCOLEfirearms proficiency certification. I further acknowledge that the following documentation has been verified by the chief juvenile probation officer and was submitted to TJJD within 30 calendar days after receiving the certificateof proficiency from TCOLE.
