Self Care - Prescribing for minor conditions

There are lots of minor conditions like colds, sore throats and headaches that get better on their own without any need to visit your GP practice. The best approach is to treat the symptoms of these self-limiting ailments with medicines that can be bought cheaplyover the counter in shops andcommunity pharmacies, usually for less than the cost of anNHS prescription.

This is better for you because you won't have to wait for an appointment with your GP to get the medication you need to start feeling better. Byself-caring, you will be helping the NHS too. We have worked out that every GP in Eastern Cheshire will have up to one hour extra a day to see patients with serious problems if we can persuade people to stop seeing their doctor about self-limiting illnesses. We will also save around £47,000 a month by limiting GP prescribing of over-the-counter medicines for minor conditions. This will help the local NHS to close the gap between the funding it gets from government and the money it needs to carry on buying the services that are available currently.

Our approach to self-care is part of our effort to encourage people to take the best possible care of themselves so they live healthier, happier lives and place less demand on hard-pressed services. Empowerment is one of the eight ambitions of theCaring Together programmeto join up health and social care services in Eastern Cheshire.

But encouraging self-care does not mean that you shouldn't seek medical advice if you feel you have reason to be concerned.

FRIENDS AND FAMILY TEST

As from 1st December 2014 if you visit our GP Surgery you will beable to complete a Friends & Family short form, stating whether or not you would recommend us to your friends and family if they need similar care or treatment.

Your answers will be anonymous and can be put in the box provided, whichare located in our waiting area.

If you have any of the following long term health conditions you will be contacted to arrange an annual review with one of our Practice Nurses during the month of your birthday:

• Asthma

• Diabetes

• High blood pressure (hypertension)

• Heart disease

• Stroke

• Chronic airways disease (COPD)

By making the appointment during the month of your birthday, we hope this makes it

easier for patients to remember when their review is due.

NHS HEALTH CHECKS

TheNHS Health Checkis a healthcheck-upfor adults in Englandaged40-74. It's designed tospot early signsof stroke, kidney disease, heart disease, type 2 diabetes or dementia. As we get older, we have a higher risk of developing one of these conditions. An NHS Health Check helps find ways to lower this risk.

How do I get an NHS Health Check?

If you're in the 40-74 age groups without a pre-existing condition, you can expect to receive a letter from your GP inviting you for a free NHS Health Check every five years. Speak to one of our receptionists for further information.

How can I improve my test results?

Once you've had your NHS Health Check,your healthcare professional willdiscuss your resultswith you. You'll begiven advice to help you lower your risk of a stroke, kidney disease, heart disease, diabetes or dementia, and maintain or improve yourhealth. But you don't have to wait untilthen to make healthy changes.

New Services

MSK services and health optimisation

MUSCULOSKELETAL HEALTHCARE SERVICES

To improve access to services for people with conditions affecting the muscles and skeleton, we have established a triage and clinical assessment service to which you can refer yourselfwithout having a GP appointment first. The triage is carried out by InHealth Ltd.

The way we offer services for musculoskeletal (MSK) treatment (bones and joints) in Eastern

Cheshire is changing. We want to give you more choices, bring care closer to your home and offer you the opportunity to be more in control. From 1 October 2017, you will have the opportunity to self-refer into these new services or be given a greater choice of treatment options by your GP.

OneYou is a free information and support service that can help you with a range of

support ranging from diet, nutrition and exercise to stopping smoking, drinking less and

Doctor McCamley will be working Monday, Tuesday and Friday. Doctor Monument will be working Monday, Wednesday and Friday.

Jack Callaghan – Physician Associate

Physician associates (PAs) are medically trained, generalist healthcare professionals, who work alongside doctors and provide medical care as an integral part of the multidisci-plinary team. Physician associates are dependent practi-tioners working with a dedicated medical supervisor, but are able to work autonomously with appropriate support.
