Player Buddy Form

If submitting Head Coach/Assistant Coach Buddy request – DO NOT submit this form.

Buddy Rules:

1)Only submit ONE Buddy Form per Player. Additional forms submitted with the same player will invalidate all forms.

2)Player Buddy Pairs MUST Request Each Other and play in the SAME Age Division.

  1. Any formrequesting more than two players per form will be discarded.
  2. All forms with thesame player listed on more than one Buddy form will be discarded.
  3. Siblings playing on the same team are allowed to each request one buddy (submit one form per sibling).

3)Both players must be registered by June 6, 2017and in the same age division.

** Please note that our division age groups have changed and are now defined by BIRTH YEAR. Even if you have had abuddypairing with someone in the past, you may now be in different divisions. Please double check the chart to be sure that the players requesting abuddypairing are in same division.**

Players may request to play in an older division but not in a younger division.

4)Buddy form must be turned in in-person at Walk-in Registration at Castro Elementary library located at 505 Escuela Avenue in Mountain View on either:

  1. Saturday, May 13, from 9:00 a.m. to noon, or
  2. Saturday, May 20, 10:00 a.m. to noon.

5)Confirmation is not a guarantee that the match will be made. Team balancing and other considerations take precedence. It is our hope to make all legitimate buddy matches that are requested in the spirit of our guidelines.

6)Request Priority is given to forms with one or more parent volunteers (coach, referee, division coordinator, board member). You will be asked for a volunteer commitment of some sort when submitting the form. AYSO is an all-volunteer organization; playing buddies are a privilege for our volunteers.
