1.INTRODUCTION

The StR Locum Appointment for Training post forms part of the West of Scotland Training Programme in Paediatric Surgery and has training approval from the Royal Colleges of Surgeons and PMETB and as such will conform to the formal training guidelines laid out in the RoyalCollege and PMETB documents.

During training the trainee is allocated a supervisor. Performance assessment will be on a regular basis throughout the post as laid out by the SAC/PMETB for contemporary specialist trainees. Summative assessment is conducted annually and is based on in-house training assessment criteria and the West of Scotland Postgraduate Committee (ARCP).

The training programme is supervised by the Regional Postgraduate Training Committee in Paediatric Surgery on which there are representatives of the TrainingHospitals, University and Junior Staff. Individual trainees are required to sign a training programme agreement and a learning agreement with their assigned educational supervisor shortly after taking up post. Study leave conforms to the terms and conditions of service for the Specialty Registrar grade.

The post provides 6 months of training in Paediatric Surgery with the possibility of extending for a further 6 months if appropriate. Training will be supervised by the Sub-Committee in Paediatric Surgery of the West of Scotland Postgraduate Medical Education Board.

This StR (LAT) post is based solely at the RoyalHospital for Sick Children, Glasgow.

2ENTRY REQUIREMENTS

The Trainee will be appointed by a Committee established by the Women & Children’s Directorate of NHS Greater Glasgow.

Applicants must have completed 2 years foundation training or equivalent, have completed core surgical training in posts approved by the Royal Colleges of Surgeons in the UK, Ireland (or equivalent) and possess MRCS/FRCS/AFRCS (or equivalent). Applicants should be able to demonstrate competencies appropriate to there experience. It is desirable to have an additional 1 year or more in a general or paediatric surgical post, or equivalent, and have 6 months experience in neonatal medicine.

Applicants will be expected to provide evidence of their competence to communicate in English. Appointment will be subject to satisfactory occupational health and Enhanced Disclosure Scotland (through the PVG scheme) clearance

3DUTIES OF THE POST

(i) Training

The Trainee must make full use of the training opportunities provided in this placement.

(ii) Clinical

Clinical Duties will involve working as a member of a surgical team looking after the in-patients, out-patients and day-patients of the relevant Consultant staff, operating and supervising the work of junior colleagues

(iii) Administration

The Trainee will be expected, when appropriate, to participate in the day to day running of the department in administrative activities such as organisation of rotas, organisation of undergraduate training, patient reports and letters. The Trainee will have the opportunity to develop management skills as required by the College.

(iv) Communication

This will include:

  • Discussion with Consultants regarding clinical problems.
  • Communication with General Practitioners - including telephone discussion of problems and writing of letters about cases seen in the Department.
  • Liaison with Specialists in other hospitals/agencies, such as district nurses, ambulance, police etc.
(v) Rota
  • The trainee will participate in a partial shift system with normal days, long days (0800-2000) and 3/4 night shifts (2000-0900). The rota has recently been updated to ensure compliance with European Working time Directive and New Deal. The rota is under regular review, as duties and commitments change the rota may be subject to revision.

4EDUCATION

(i) Continuing Education

Management development is encouraged and the Trainee will be expected to attend appropriate courses.

(ii) Teaching

This post has a commitment to the teaching of undergraduate medical students during university term time. The registrar is also expected to take an active part in the postgraduate educational programme of the hospital and the Directorate; this includes regular weekly audit and clinical meetings, radiology, pathology, oncology, renal, nutrition team, foetal medicine meetings and a journal club. The RoyalHospital for Sick Children has a full educational calendar throughout the year and includes weekly Grand Rounds, departmental meetings and guest lectures/seminars. The hospital has an active Open Learning Centre and a Computer Cluster teaching facility with internet access.

(iii) Library Facilities

There is a good medical library in the new Forrester Cockburn Postgraduate Centre. This facility also boasts a computer cluster, which is accessible by all hospital staff. There is an active Learning Centre within the hospital. Surgical textbooks and journals are held for members of the Directorate in the Library in the combined neonatal intensive care unit. There are also some relevant reference texts in the Accident and Emergency Department.

(iv) Postgraduate Medical Education Courses available in the locality

Lectures in the Royal College of Physicians and Surgeons of Glasgow, the University of Glasgow, the University of Strathclyde and Western Postgraduate Centre (Lancaster Place) take place on a regular basis (see published calendars for details). Special Courses and Seminars are arranged from time to time. Formal in-post training is arranged by the Surgical Directorate, the Postgraduate Tutor and by the Yorkhill Medical Staff Association. Attendance at PALS and ATLS courses is encouraged.

5RESEARCH AND AUDIT

(i)Research

There is a strong tradition of research in the West of Scotland. Facilities and support for research activities are available through the University academic department and within hospital departments. The trainee will be encouraged to participate in clinical research and time will be allocated consistent with current Terms & Conditions.

(ii)Audit

Audit is an essential component of Paediatric Surgery practice and the Trainee will be required to participate in local and national audit activities.

6LOCATION OF DUTIES

Duties are entirely within the site of the RoyalHospital for Sick Children, Yorkhill.

7ASSESSMENT

(i)The Trainee will be continuously assessed by the trainers in their place of work with formal interviews every 3 months. Competency based assessment schemes (work based assessment tools) are currently in use complying with the SAC and PMETB requirements.

(ii)If in the opinion of the Training and Assessment Committee, the Trainee fails to show adequate commitment or progress, career guidance will be arranged, and exceptionally, in the absence of satisfactory progress, the committee may recommend that the contract be terminated.

(iii)Trainees are required to keep a logbook following the requirements laid down by the Royal Colleges of Surgeons.

8GENERAL INFORMATION

If appointed as a Clinical Fellow this post will not be recognised for Training

The hours of duty will be the standard working week of 40 hours for which a standard salary (as detailed in paragraph 8b, Appendix 1 of the Terms and Conditions of Service, appended under general information) will be paid.

From December 1st 2000, a new system for calculating Junior Doctor's out of hours pay was introduced, the 'Banded Intensity Contract'. The standard hours of duty will remain 40 hours per week and in addition a supplement will be paid. The banding for the hospitals on the rotation will be confirmed on appointment.

Please note that following a recent change to a partial shift system a monitoring exercise is currently being undertaken, therefore, the current Banding may be subject to change. An offer of Employment may not reflect the current Banding.

ROYALHOSPITAL FOR SICK CHILDREN, YORKHILL.

Brief Description of Hospital

The RoyalHospital for Sick Children (RHSC) is the main teaching, tertiary referral children’s hospital serving the West of Scotland. It has a complement of over 200 in-patient / day care beds and there is additional accommodation for parents and accompanying adults.

RHSC is a Teaching Hospital of the Faculty of Medicine of the University of Glasgow.. University Divisions based at, or involved in, the RoyalHospital for Sick Children, include the Departments of Child Health, Paediatric Surgery, Medical Genetics, Human Nutrition, Child and Adolescent Psychiatry and Clinical Chemistry.

The Work of the Department

The Surgical Directorate comprises the following specialities:

There are four Surgical Wards and a combined neonatal intensive care and infant ward. Ward 3a admits surgical specialities including orthopaedic, plastics, ENT and opthalmology patients, ward 4b is the acute surgical receiving unit together with elective general surgery. Urology is accommodated in the combined Nephrology/Urology unit in ward 6a. Ward 5a is the Cardiology and Cardiothoracic surgery ward and the complex airway patients. Neurosurgical patients are managed in the neurology ward primarily under the care of the paediatricians. In addition, there is a busy Day Surgery Unit with its own ward area directly adjacent to the theatre complex. There are 7 fully equipped theatres (including one OR1 integrated laparoscopic theatre), an endoscopy suite and a dental procedures theatre. The Surgical Directorate has very strong support from Laboratory Services and from the Department of Diagnostic Imaging.

The Hospital has been designated as the Scottish National Centre for Paediatric Cardiac Surgery and has designated national funding for extracorporeal life support, renal transplantation, molecular genetic services, PICU transport, neonatal biochemical screening and endoprostheitc replacement following major bone tumour surgery.

The hospital also has a new integrated high dependency/intensive care unit of 22 beds. There are also moves to centralise of all paediatric accident and emergency services these changes have required a review of accommodation and staffing in the Emergency Department which is still in development.

The Surgical Directorate provides core surgical services for the children in the Greater Glasgow Health Board and also acts as a secondary and tertiary referral centre for the West of Scotland and further afield. The extracorporeal life support (Scottish ECMO) service, which is based in the Surgical Directorate, offers a service to children throughout Scotland, the rest of the UK and overseas.

The University Division of Surgical Paediatrics organises an active teaching programme throughout the year and there are many opportunities for clinical and laboratory research within the department.

The Surgeons are organised in teams involving Consultants, Registrars. The LAT will work in one of three teams for a three to six month periods. During higher specialist training in Paediatric Surgery at Yorkhill, the higher surgical trainee will gain experience of the whole spectrum of neonatal and general paediatric surgery, paediatric urology, oncology and neonatal critical care. Research activity is encouraged and, indeed, expected of higher surgical trainees while in this post.

The post will involve close liaison with Nursing, Secretarial and Ancillary Staff and the Professions allied to Medicine. The Training lead supervising the educational aspects of the post is currently Mr Martyn Flett. TERMS AND CONDITIONS OF SERVICE

The conditions of service are those laid down and amended from time to time by the Hospital and Medical & Dental Whitley Council.

- If appointed as an StR (Locum Appointment for Training) the salary range will be: £29,705 to £46,708.
- If appointed as a Clinical Fellow the salary range will be: £29,705 to £39,300.
New Entrants to the NHS will normally commence on the minimum point of the salary scale, (dependent on qualifications and experience). Salary is paid monthly by Bank Credit Transfer.
