Affiliate Meeting Minutes

CDDO Updates:

·  July Affiliate Report – The monthly report was handed out and reviewed. There are currently 950 individuals receiving services in Shawnee County. Ramona noted that the CDDO has been averaging more than 10 referrals per month for DD eligibility determination. In July, 4 individuals were determined eligible for services and there are 6 more pending a determination. The fiscal year began July 1st; therefore the year-to-date number of crisis requests approved for the year is for 2 out of the 5 crisis requests reviewed in July 2007. Provider changes reflect that individuals are choosing a variety of providers for their day services so they may spend several days of the week in different settings rather than one provider for every weekday; Case Management changes are higher at 17, which may be attributed to a provider ceasing to provide case management services. There are only 69 individuals in Shawnee County who have not selected a case manager that the CDDO provides basic case management functions to. This number is much lower than several years ago.

·  Provider Emergency Plans – At the July meeting, there was a group of providers who had expressed interest in working together on an emergency plan in the case of natural disasters. Individual providers had indicated they had some back up planning in place and would also be relying on other entities such as the city to provide temporary shelter. Mary Ann Keating, TARC, and several others (none were able to attend today’s meeting) plan to begin discussion toward logistics for working together to develop a plan. ·  Case Management Conference – Ramona reported on the success of the 2nd Annual Case Management Conference which was held in July. Case Managers from all areas of the state serving the developmental disability and mental health fields attended. In all there were 242 paid attendees, 39 presenters (including 2 keynote speakers – Dr Walter Kisthardt, PhD, MSW and Dr Michael Mayer), 15 conference planning committee members and 14 volunteers. The planning committee members consisted of representatives from the following case management agencies: Dream Catchers, Individual Support Systems, Monaco & Associates, Occupational Center of Central Kansas (Salina), Sheltered Living, Sunflower Supports, TARC and Valeo Behavioral Health Care.

·  Risk Assessments – A copy of the handout provided at the August TCM meeting by SRS Quality Improvement staff was shared and reviewed. Ramona noted the handout provided good examples and information. Some highlights include: Items that may be included in a PCSP; signed voluntary informed consents are needed in the persons file when psychotropic medications are prescribed, along with a statement of side effects of the drug. It was also noted that SRS is comparing the behavioral section of BASIS to the individuals PCSP per KAR 30-63-21 to see if these areas are identified along with how the person is to be supported.

·  Hot Weather Reminder – Please remind staff to not leave individuals unattended in vehicles for any length of time. The vehicle can quickly become very hot and individuals who already have health issues can be adversely affected by the heat quickly. Ramona noted there have been a couple of incidents where individuals have been left alone in vehicles and wanted to remind everyone of the heat danger.

HCP/CSS News:

·  SRS Money Follows the Person Grant – An SRS memo announcement was handed out with information that Angela Reinking was hired for the Project Director of the Money Follows the Person program beginning August 13th. The notice also announced Lauren Otto as the Real Choice: Systems Transformation Grant Project Director.

·  SRS/CDDO Contract Negotiations – Ramona announced that the SRS/CDDO contract mediation will be held on Thursday, August 16th, at the Ramada Downtown.

·  SRS/CDDO Business Meeting – Ramona announced that the SRS/CDDO Business Meeting is scheduled for September 18, 2007 at Jayhawk Towers in Topeka. The Business Meeting is an open meeting.

Other:

·  Special Journeys for Special People – A brochure was provided by Special Journeys offering upcoming events and cruise information.

·  Case Management Conference – There were a variety of handouts left over from the Case Management Conference which were available to take if interested.

Info/updates/resources/trainings:

·  A handout for the upcoming CDDO Quarterly trainings on August 23rd was provided. Sessions include: CDDO Overview, Article 63/Licensing, BASIS and Beginning Plan of Care.

·  CDDO Lunchtime Movie & Discussion – the next session will be held on August 24th and the movie “Goin Mobile” will be viewed and discussed.
