Enrollment Agreement
In the event that I am notified that my child is in need of being picked up at the center for health or other reasons I agree to make arrangements for my child to be picked up within 45 minutes. Failure to do so can results in my child’s termination from UMCC.
I agree to have all pre-admission forms completed before my child starts the program. I understand that children will be released only to parents or people designated by parents in the pre-admission forms. Parents must notify the center of any change in arrangements in writing.
UMCC is open from 7:30-5:00. Please plan accordingly and arrive at 7:30 or after and pick up by 4:55 in order to pack up all belongings.
I understand that a $25.00 late fee will be assessed for every ten minutes of tardiness after the hour of closing (5:00p.m.). Repeated violations in this area can result in my child being terminated from the program. You will be billed for this fee.
During this time period, it is understood that Undermountain Child Care will be closed for specified holidays, in-service and any snow or necessary but unexpected days and for a week during summer for maintenance purposes (see 2016-2017 calendar). Unexpected closings are not refunded.
Permission Check List
Emergency Evacuation
I give permission for my child ______to travel off campus with the UMCC group and his/her teachers in the instance of an emergency evacuation or field trip. I understand that I will be given adequate prior notice to planned trips.
Class List
______Please check if you want your name and telephone number included on the class list to be shared by all the families in UMCC. This information will be used for invitations to parties or play dates.
Photographs
______Please check if you authorize the use and reproduction of these photographs for UMCC use.

Creams & Lotions
All products must be brought in by the parent and will not be applied without written permission.

Tooth Brushing
______Please allow my child to brush his/her teeth.
______I am choosing to “opt out” of the tooth brushing program for my child.
Parents are required by the Department of Early Education and Care to supply a new toothbrush every three months or after an illness when participating in our Oral Health program. Requirement 606 CMR 7.11(11) (d)
