Upon receipt of your signed offer letter a representative of the College of Arts and Sciences Business Services Center (BSC) may contact you. The BSC representative will evaluate all available existing records and provide you with any required new hire/rehire forms. This offer is contingent upon completion of any/all new hire/rehire requirements, which include:

1. Employment Eligibility Verification (I-9) Form: The Ohio State University is required by federal law to verify the identity and work authorization of all new employees. Accordingly, this offer is contingent upon verification of identity and eligibility to be employed in the United States.

2. Statement Concerning Your Employment in a Job Not Covered by Social Security: Ohio State University faculty, staff and students participate in state retirement plans, rather than in the federal Social Security program. This form explains the implications of having this type of retirement plan should you also have entitlement to benefits under the Social Security program from your or a spouse’s other employment.

3. Ohio Ethics Law Acknowledgement Form: Ohio State faculty and staff are covered by the Ohio Ethics Law and Related Statutes for public officials and state employees, and accordingly must receive and acknowledge a copy of this legislation.

4. Acknowledgement of Receipt of Auditor of State Fraud Reporting System Information: Ohio Revised Code 117.103(B) (2) requires the university to make all faculty and staff aware of the Auditor of State’s new fraud reporting system established by this statute, and accordingly you must receive and acknowledge a copy of this legislation.

5. This offer is also contingent on the university’s verification of credentials and other information required by law and/or university policies, including but not limited to a criminal background check. You will receive an e-mail message requesting the information required to initiate the background check process, if applicable.
