MGHCA Nomination Form Instructions:

In general…

1)Please do not change any formatting or fonts on the spreadsheet. We nomination forms are received, we simply cut and paste the information onto a master spreadsheet. If the data is not all in the same format, information gets placed into the wrong cells.

2)Please don’t bold or use all capitals, I think I’m obsessive compulsive and I have to go in and change it all after.

3)You only need to enter a players information once. If you are nominating them for more than one thing (Ex. All-State, Advanced 17’s and Jr. Festival) just put an X in the appropriate columns.

4)When nominating players for…

  • All State… we are looking for the “best of the best”. If you are not sure what kind of season your player(s) might have, nominate them now and pull their names off the list later if they don’t play as well as you had hoped. These nominations will go out to all Head Coaches on a Preliminary Ballot and eventually on to a Final Ballot that goes out to the selection committee (Executive Board and Section Representatives).
  • Senior Classic… we are looking for players on your team you think are capable of playing college hockey (D1 or D3). These nominations will be submitted to the GM’s for each Section (A & AA combined) and will be voted on by Section Coaches.
  • Junior Festival… this is a tryout process, so feel free to nominate all of your Juniors. Tryouts are held in conjunction with the Advanced 17 tryouts, but players need to apply for each program separately.
  • Freshman/Sophomore Development Camp… this is a first come, first serve program. We would like to open it up to Varsity Players first, but if spots are available, we will open it up to top end JV Players. Please indicate in the Fres/Soph column whether the player is a Varsity Player (V) or a Junior Varsity Player (JV).
  • Advanced Programs… these programs are Minnesota Hockey Programs, but the Phase 1 portion of the 16’s and 17’s are run by the MGHCA for Minnesota Hockey. The 15’s are run by Minnesota Hockey at the District Level. Please nominate all of your Varsity kids who fall within the specified birth year.

15’s are kids born in 1996

16’s are kids born in 1995

17’s are kids born in 1994

The goal of Minnesota Hockey is to have 40 kids trying out per section per age group, so in some Sections, JV players may be invited to tryout as well. Please indicate in the Advanced Program column whether the player is a Varsity Player (V) or a Junior Varsity Player (JV).

Nominate your 17’s, 16’s and 15’s here. We will pass the Advanced 15 nominations onto Minnesota Hockey, but you may here from your local District GM as well asking you directly for a nomination.

When entering player information…

5)First Name… please use the first name of the player as it appears on their birth certificate and put their nickname in parenthesis after it. Example: Samantha (Sam)

6)Class… please enter the year the player graduates (not their current grade level). Example: 2010

If you enter 12, we don’t know if that means she is a Senior or that she graduates in 2012

7)Birthdate… please enter as mm/dd/yy

8)Parents names… please enter parents last names if they are different from each other or different from the player. This is information that we need administratively if we need to match up checks with players.

9)Cell Phones… from past experience, when parents fill out our applications, they usually list cell phone numbers instead of home numbers, so it appears they would prefer to be contacted this way.

10)e-mail Addresses… THESE ARE THE MOST IMPORTANT COLUMNS. Most of our correspondences will be going out electronically. It save us a lot of time and money. Please make an effort to get at least one e-mail address for each player. The more e-mail addresses we have the better. Players who provide e-mail addresses will receive information and invitations much sooner than players without e-mail addresses.

11)DEADLINE:

I’d like to get nominations by November 30th so we can send out a Preliminary All-State Ballot before the Holiday Tournaments.

This form will (hopefully) remain standard, so if you save it, you can just delete the previous years Seniors and add in any new players to save yourself some time. If you want your last years completed nomination form, I can e-mail you a copy.
