Improvement & Support Workstream

Dear Colleague

Building on the work of the TEC programme, we are offering funding,up to £15,000,for around 6technology enabled care Tests of Change. This offer is open to any NHS Board, Integration Authority,Housing, Independent or Third Sector organisation and is intended, primarily, as asupportto inform local or organisational planning and commissioning activity.

The aim of any application must align with the overarching TEC programme aim which is to support more citizens to make greater use of technology to manage their own health and wellbeing at home and in the community. A test of change should seek to demonstrate measurable improvement in outcomes either directly to individuals or indirectly through improved service delivery processes.

Applications for this offer must complete the attached Improvement Charter, outline the detail of funding requested and meet the following requirements.

  1. The setting out of a clear and objectiveImprovement Aim is essential.
  2. The Technology or TEC service involved must be citizen focussed (e.g. Telecare, VC, HMHM, digital applications)
  3. The Test of Change Charter must demonstrate a clear rationale for the proposed test of change;outlining the expected benefits and how improvement will be measured.
  4. By submitting an application, partnerships are confirming that they will be in a state of readiness to initiate their proposed test of change by the end of January 2017with learning feasibly concluded within a further 90 days.
  5. Readiness includes the ability to free up appropriate resource to complete the process if required.
  6. Readiness also infers that there is organisational support for your change and willingness to support proposed change should it be successful.
  7. Funding can be sought for low cost “nonstandard”equipment or associated Technology costs(i.e not standard equipment already in use by your service). This may be about testing “new to you” technology, applying technology in a different way or at a different time in a pathway or testing out a model which other partnerships have found successful.
  8. Funding for staff time can be sought but must not be for operational costs, nor reliant on any form of recruitment process. (Cost may be for e.g. backfill, Clinical or other leadership, analytical support, improvement support or project support for duration of improvement project).
  9. The proposed change must not already be supported via current TEC programme funding or other funding stream.

Successful applicants will be expected to share their progress and learning through monthly, one page, storyboard updates,a completion report and Webex presentation.
