Comments:
All engineers undertaking this type of work must be fully trained, competent and familiar with the equipment involved. Anyone under the age of 18 must not use this type of equipment unless they are undergoing training and are under constant supervision.
Hazards
1.  Falls from platform.
2.  Mechanical failure of a load bearing component
3.  Falling materials.
4.  Platform or truck striking pedestrians or fixed objects
5.  Platform or persons striking items fixed overhead
6.  Platform could be struck by other mobile plant or equipment
7.  Adverse weather or extremes of temperature (Cold store -25)
8.  Staff could become trapped in an emergency if the truck driver abandons his position.

10.  Platform and truck may tip over if driven with platform elevated or used on rough, uneven or soft ground

(Note a restraint lanyard is designed to prevent a fall by restricting the wearers movement to within the platform. longer lanyards (fitted with shock absorbers) may allow a person to fall from the platform, then, dependant upon the working height of the platform may not deploy and arrest the fall before the wearer hits the ground.)

Equipment / 1.  Truck and platform must be suitable for the conditions (-25ºC, rough ground, etc).
2.  Truck and platform must be inspected every 6 months (LOLER requirement) ask for proof.
3.  Max load should be marked on the truck and platform.
4.  Platform floor should be free draining.
5.  Guardrails and toe boards must be in place.
6.  Gates should open inwards, upwards or sideways, and close and lock automatically.
7.  Platforms should have safety harness anchorage points.
8.  Platforms must have guards to separate people from the moving parts of the truck.
9.  The platform must have fork pockets and a positive locking device to secure it to the truck.
10.  Do not use trucks with erratic movements.
11.  Do not use trucks with capacity of less than 1000kg.
Information, Instruction, and Training / 1.  Ensure that the driver of the truck is appropriately trained (ask for evidence e.g. CITB licence).
2.  Do not overload the platform, stay within the manufacturers safe working load. The weight of the platform together with its load of people, tools, materials, etc should not be more than half the actual capacity of the truck.
3.  Agree a method of communication with the driver (E.g. Radio, etc).
4.  The driver must not abandon his position whilst the platform is raised.
5.  Persons must wear a safety harness and safety lanyard. It is recommended that the lanyard be attached to the platform at all times. However, if the platform is stationary it is permissible not to have the lanyard attached, providing you do not need to lean out.
6.  DO NOT attach your lanyard to a fixture of the building or equipment being worked on (Support steel work, evaporator, etc) as you may be dragged from the platform if it moves away.

8.  Do not allow the truck to be moved with the platform elevated.
9.  Ensure adequate ventilation if using a diesel powered truck indoors.
10.  All staff required to wear Harnesses must be trained in their use and maintenance.
11.  Ensure that platform guardrails and toe boards or other suitable barriers are in place.

1.  Periodic checks are made to ensure that control measures are being adhered to.
2.  Appropriate PPE is used.
3.  Our subcontractors adhere to the requirements of this risk assessment.

2.  Only use the truck and platform on firm and level ground.
Procedures, documents etc / 1.  A checklist/permit to work accompanies this assessment; completing it will assist you to comply with this risk assessment.
2.  A client may have more stringent rules regarding the use of Fork-lifts and platforms on their site, these must be followed in addition to our rules.

Emergency action/procedures / 1.  Ensure that everyone knows what to do if the machine fails in the raised position. Ensure that you have some form of communication in case the equipment fails, particularly in low temperatures.
2.  Ensure that the driver of the truck does not abandon you. He is your only means of escape in an emergency
Access / 1.  Agree barrier provision and road or access way closure.
2.  Alternatively agree a safe operating area.
3.  Cordon, if necessary, using cones or tape, an area beneath the platform to prevent pedestrians being hit by falling objects.
4.  Prevent vehicular access in the vicinity of the platform in order to prevent collisions.
