Clinical Competency Tool (CCT)
CCT Instructions and Submission Directions:
The CCT documents student success in clinical and is included in the student file for graduation requirements. Below are detailed directions for completion and submission of this important document. PLEASE NOTE: By the end of the community clinical experience, the student must successfully pass the following critical competencies in order to pass the course. Competencies must be performed accurately and safely. If this document is not received by the course faculty at the end of the course, the student will receive an I/F.
Student: Please fill in your name, date, and course faculty name and faculty Regis Net email address before sending this form electronically to your preceptor as an attachment (do not give as hard copy). Review and sign the CCT with your Preceptor (use electronic signature; your signature is not necessary but nice to have on the form). You will then upload to the NR 478R assignment dropbox and send to either for campus based students or to for online students.
Preceptor: Pleasereview this document at the beginning and end of the clinical experience with the student. This document should be completed online and signed electronically by Preceptor (and Student if possible) for ease of sending as attachment by email. At the end of the clinical experience, attach the completed and signed CCT as an email attachment and send to the Course Faculty. (The student will give you the name/email of the course faculty.) IF you are unable to email the document, please contact the course instructor. (For confidentiality reasons, once you have confirmation from the course faculty that the CCT was received, please SHRED/DELETE this document.) . Thank you for being the student’s preceptor. It is greatly appreciated!
