Citizen of the Year Nomination

1. The recipient may be either a man or a woman and reside in Lenoir County. If a man and woman have performed outstanding services during the course of the year, each may be awarded a plaque. (A couple may be nominated.)
2. The recipient of the “Citizen of the Year” Award must be over the age of 40.
3. Once a person has been awarded the “Citizen of the year” Award, he or she will not be eligible again for this award.
4. The person chosen for the “Citizen of the Year” Award will have rendered outstanding services for Lenoir County. The limitations of this service are not defined because meaningful service to the community is often long range. However, special emphasis should be given to services performed during the current year.
5. The services rendered do not have to be connected with the Chamber of Commerce programs, nor does the recipient have to be a member of the Chamber of Commerce.
NOMINATION PROCEDURES AND RULES
· Complete the nomination form. You may choose to attach it as the cover page to accompany letters of explanation and/or recommendation or supporting documents.
· All nominations must be submitted in a sealed envelope marked with the award name (example – COY Nomination enclosed) and mailed by either certified USPS mail or hand delivered to the Kinston-Lenoir County Chamber of Commerce, 301 North Queen Street, Kinston, NC.
· All nominations must be postmarked or delivered to the Chamber by the deadline date and time.
· Nominations delivered to the Chamber must be delivered to a Chamber employee.
· All nominations will be kept confidential.
· The contact person listed on the nomination form will be notified if their nominee is chosen by the selection committee

Describe local, county and state level participation, length of service and offices held in civic and service organizations, charitable activities, political pursuits, religious groups, chambers of commerce, merchant’ associations, etc.
