The undersigned insert name of signatory of this form declares that the data below correctly represents the average of the number of reference materials distributed by insert full official name of the legal person :

Data are given as the average number of units distributed per yearin the fiscal years 2016 and 2017 of reference materials produced by the following institutions:

  • Bundesanstalt für Materialforschung und –prüfung (BAM), Germany
  • Centro Nacional de Metrología (CENAM), Mexico
  • European Commission/Joint Research Centre/IRMM, Belgium
  • Instituto Nacional de Metrologia, Qualidade e Tecnologia (INMETRO), Brazil
  • International Atomic Energy Agency (IAEA), United Nations
  • Korea Research Institute of Standards and Science (KRISS), Republic of Korea
  • LGC Ltd, United Kingdom
    This refers to the NMI Branch of LGC, i.e. certificates signed by the UK Government Chemist
  • National Institute of Standards and Technology (NIST), USA
  • National Metrology Institute of Australia (NMIA), Australia
  • National Metrology Institute of China (NIM), People's Republic of China,
  • National Metrology Institute of Japan/National Institute of Advanced Industrial Science and Technology (NIM/AIST), Japan
  • National Metrology Institute of South Africa (NMISA), Republic of South Africa
  • National Research Council of Canada, Canada

Pure standards (solid or in solution) are attributed to the groups as follows:

  • Pure standards that are only relevant for analysis of food (e.g. mycotoxins) are included in the group "food materials".
  • Pure standards for PAHs, PCBs, pesticides etc. are equally distributed over the groups "Food Materials" and "Environmental Materials"
  • Pure standards for trace metal analysis are equally distributed over "Food Materials", "Environmental Materials" and "Industrial Materials".

The above-mentioned person may be subject from rejection from this procedure and to administrative sanctions if information provided as for awarding of the contract prove to be false.
